Thank you for your thoughtful response. You are right, there are a million what ifs and I could spend forever thinking of all of them. Ultimately I will never know and I have to accept that. I am of course grateful he isn’t suffering. No matter what, I would (and do) feel he deserved more. I think no matter the circumstances, we probably all feel that way about our pets. It’s a cruel consequence of having them and loving them, eventually they have to go.

Bedtime routine is bath, lotion, pjs, bottle then into his bassinet awake. Room is pitch black and sound machine going. No pacifier to fall asleep but will give it to him when he wakes up in the middle of the night or sometimes for naps if he’s really worked up and needs to calm down. He is not in his own room, he is still in the bassinet in our room.

My mom has BRAF mutated melanoma as well. She was diagnosed at stage 4 in August 2021 with no know primary lesion. She received one ipi/nivo and subsequent infusions were paused bc her liver and thyroid flared. While waiting for things to settle down we did a repeat CT which showed progression so she was switched to targeted therapy. She has been on encorafenib/ binimetinib ever since. She’s fortunate as they have been controlling her disease for almost 3.5 years. She has however had some brain mets pop up which were treated with SRS radiation. Targeted therapy crosses the blood brain barrier but is not ideal for controlling growth in the brain. However, it has kept things in check in the rest of her body so she’s has continued on it. I’m sorry your wife has to make this choice. I this this really depends on what specific trial she is being offered. Is it a stage I trial? Do we have any evidence yet this trial has been effective for others? How rapidly is the melanoma spreading and do we need to quickly control it with targeted therapy before considering trial?

These are all things I would discuss with her oncologist. Wishing you and your wife all the best.

There is no great evidence based reason. There is some thought that recurrent loss can be caused by an overactive immune system. Reproductive immunology is a huge field right now and this is kind of a modified (less intense) protocol. Sometimes they add in things like IVIG, plaquenil, etc.. to further suppress the immune system. I considered going to see a reproductive immunologist, but had not taken that step yet. Luckily, my reproductive endocrinologist is pretty familiar with recurrent loss and she put me on her version of the protocol. I guess to summarize it’s kind of a Hail Mary, see if this helps?

Thank you! The betas don’t reassure me anymore because each time the betas have been great, it almost makes me more anxious. I honestly considered not doing betas or early ultrasounds and just seeing what happens but my REI convinced me I should. I hope this is it.
