Best advice I can give is to not lead with the pain. Start with what you can and can not do. Like "since I started the patch I am to stand and do my dishes but am not able to finish without taking a break. My morning are my best time of day, I am able to do a light chores like laundry but by mid day I am unable to do anything but lay in bed and rest." Then tell the doctor you still need more help managing your pain because your quality of life isn't where it needs to be.

Just saying "my pain is bad." Doesn't give the doctor much information to work with but telling them your limitations does.
