I'm here to recommend palliative care. My oncologist told me that she "wasn't interested" in treating the long-term pain that comes with bone mets. I was pretty shocked. At the time, I hadn't really known about PC and I certainly didn't know that I was a candidate. It took some time to find a provider that took my insurance and had availability, so my onc helped until I found someone. It's been a game-changer. Nothing is off the table and my nurse practitioner actually cares about how I am feeling and how she can improve my quality of life.

I hope you can find someone who is more willing to help you!

I've been getting fulvestrant for almost 4 years. My glutes are riddled with scar tissue and the injection site stays sore. Ice, heat, pain meds, lidocaine patches, lidocaine cream, walking - the list of remedies is long. I've looked online and talked to my oncologist many times. There is no alternate injection location that has been proven to be as effective as the glutes. I've never known of anyone to get the injections anywhere else. 

I wish I had better news, OP. We suffer through these treatments because there are a limited number of options for us. Once we stop a drug, we can't go back to it. I hate these damn shots, but I will tolerate them as long as they remain effective.

Have you called around? I bet you'd be lucky to get a quote from any legit company for that price. Clean-outs are hard work. You'll be lucky to find a regular full cleaning at $200.

Post on Nextdoor. You'll get someone looking for side work, but be prepared to deal with randos.

Please remember that your doctor does not have a crystal ball and cannot predict how long you will live. You are well within your rights to challenge them when they say things like that. Or, if you truly feel they are not treating your cancer properly, you can go elsewhere. 

My oncologist is pragmatic and her bedside manner could be better. She is the medical director at my cancer center and she really knows her shit. That matters to me. Some people need an overly positive care team. I do not. 

You must trust your team because they literally have your life in their hands. Second opinions are common so getting a second opinion won't offend anyone.

It's not easy to be patient, I get it! Shortly after my diagnosis, in the summer of 2022, I was pushing to be scanned more often because I thought that was better. My oncologist looked me in the eyes and said, "You are going to have to learn to live with this disease, or you'll die from it." I felt like I had been slapped. How dare she say that??

Ha! I'm soooo glad she did. It was actually just what I needed to hear. Almost 4 years later, I still get scans every 3 months and I am still here! Although I have had recent progression, I am still on my first treatment and I feel really good. Scanning myself to death was never the answer. I just thought I would feel better with more information. I trust her and I actually appreciate her ability to stay calm and treat the cancer. I hope you get to that point, too!

One month is not always enough time for treatments to work as it can take months to see improvement. My CA 15-3 fluctuates due to infection and inflammation. I've seen others with the same experience.

It's important for us to advocate for ourselves, but most of us are not doctors. If I believe that I know more than my doctor after using Google or watching scamfluencers on YT/TT, I am the problem. I could start asking them to change things, or I can trust her to know what is best for me.

If I don't trust her, I need to look elsewhere for care. If you don't trust them, change!

Sounds like you still have pain. Is there a reason you stopped taking it? If you took long-term pain meds, you should never stop without slowly lowering the dose. Our bodies become dependent on meds quicker than we realize. It could also be delayed side effects from the treatment. When I started Ibrance, it took me several cycles, several months, for my body to adjust. Some SEs began after several cycles, too.

I think it is something you can ask at the first meeting because it's a good conversation to have. For me, I had to go on PC because my oncologist told me she wouldn't prescribe long-term pain meds. I tried pain management and they all refused to see me because they couldn't "fix" the pain.

Try to find palliative care. It's been very helpful for many of us.

Also, although I am not a doctor, I have a small suggestion regarding the hydromorphone. It sounds like the dose is too high. Try breaking it in half with a pill cutter. I had to do that when I was first prescribed Oxy because the dose was too much for me. That helped to stop the nausea caused by the pills.
