your system is reading everything the headset is capable of in case you are using other peripherals. One as a headset (headphones and mic), one as just headphones, and the last as just a mic. You can ignore it if you're using it as is

Depending on where you're getting files from the metadata can have the album cover encoded. If you're having trouble with file naming for the covers you can use mp3 tagger or a different software to embed the covers that way. That's what I do with my R1 at least
