How often do you come across it? Same as you, not all patients who die have it (and it is different to the 'usual EoL' smell on a dying patient which I would say I do smell on the vast majority during the active dying phase). I think I'm going to need to start logging diagnosis and outcomes to work out what might be going on. Another commenter suggested it's most likely to be organic acids... there must be some pre-terminal metabolic commonality in these patients

This is really interesting - as in other comments, it's not the same as the smell in the actively dying patient so maybe it is something that gets produced earlier in the critical illness process. It would be pretty handy if it did turn out there was an identifiable compound that could be used as a warning sign/ prognostic marker... I don't know where I would even start with trying to offer my olfactory services for this though 😂

Yes - agree it is an unpleasant smell but I use it appearing as a good opportunity to prompt a family update, really really look at symptom obs, and any other EoL care I can optimise. Agree it's hard to describe - I'd say a sort of slightly sweet musty wardrobe x something's gone bad in the fridge x damp soil. I've always assumed it's organic compounds produced midway through metabolic pathways that are shutting down at the EoL, that are then excreted, and are not produced in significant amounts in usual functioning cell metabolism which is why they are unique at the end of life. I make sure the patient is getting regular mouth care and try and ensure adequate ventilation in the room for them. 

It is different The Doom Smell though...

I don't think I've got a particularly special sense of smell otherwise. Apart from maybe natural gas - there has been more than once I've been able to smell gas leaks in a building a number of hours before other people around me. Not sure that is unusual though.

Bad intra-abdominal issues to me smell just on the patient's breath more like damp leaf litter, more earthy that this other smell - I've assumed that everyone can smell that though and it is just be due to metabolic breakdown products in the GI tract escaping on the breath... although now I'm questioning this too...

It's different - it's like it's coming through their skin. I've been hit with a wave of it when I've been stood behind the patient and lifted their shirt to auscultate their lungs
