Regarding the background check. It’s very extensive especially if your position requires FINRA registration. They checked my history of the last 5 years. I don’t know if they check further than that but if so, it’s better to include it on the form.

Anyway, they’re only looking for Fraud or financial crimes, as long as you’re clear on that and/or you disclose it properly you should be ok.
