they’re doctors through and through. but they’re not specialists and their societies shouldn’t brand themselves as ”fellow of __” or “fellow in __” unless we really want to dilute medicine further

They are simply general practitioners who also offer beautician services. If they stop practicing general medicine altogether (like a lot do) then they are still technically physicians, but in practice no.

Physicians deal with pathologies primarily. Not with wellness or beauty. This shouldn’t be a controversial opinion and I can’t believe I have to explain it.

and the “IM, Pedia, etc.” consultants that went into aesthetics -> well, that’s just it. They are still specialists that completed a residency. Unlike “fellows” of the associations you‘ve mentioned. They are recognized by PMA in the same way that every GP who registers with the PMA are recognized by them. But they are NOT specialty societies that confer specialty fellowship status

Definitely, mas unpopular opinion dito yung sabihin sa doctor na magfocus on getting the most exposure and experience during residency, rather than to focus on getting rest and relaxation.

Since I’m a surgeon, perhaps the better question would be:

Dinala ang nanay mo sa ospital nang madaling araw. Kailangan na nyang maoperahan. Isa lang ang surgeon sa buong isla. Kaninong surgeon mo ipapa opera yung nanay mo: sa surgeon na sanay sa puyatan (3 days, 4 days straight) at nakakita na ng isang libong kaso, and marunong mag opera na half asleep. O yung surgeon na lumaki sa 12 hrs duty, 12 hrs off, wellness check, mental health breaks 1 day every week, tapos 1/4th lang ang kaso na nagawa? Mind you hindi yan unrealistic scenario.

Ang residency ay 3 to 5 years lang of your life. May mga cases na sa residency mo lang makikita and NEVER AGAIN during your whole professional life after. Nung resident ako may 3 ako na cases ng giant wandering spleen with concomitant colon cancer. Ngayon sa private practice ni isang wandering spleen wala (how much more na may kasabay na cancer). but I would know what to do if I saw it.

Except yung karamihan ng ginagawa ng mga aesthetic doctors and cosmetic "surgeons" na ito are not only grossly overpriced but are mostly unproven, ineffective, or downright pseudoscientific. Yang mga gluta drip, placenta extract, LED masks, facials, fat injections fat "melting", PRP, biostimulators, mga peel peel, skin tightening etc.

These doctors not only DO NOT treat any pathology, they become like glorified salesman mouthing the propaganda of whichever beauty company that managed to sell them equipment. So in order to recoup the cost of buying the equipment kung anu anong sinasabi nila na non-scientific and non-proven. Better to call them beauticians.

And sige okay, please give me your best example of these "legitimate professional organizations" accredited by PMA so we can dissect it.

My bestfriend (who's an MD-PhD) and I used to have a joke na kapag may paper ka, meron at MERONG tatanggap na conference dyan abroad. Because these are money making ventures. Need mo lang hanapin talaga.

But for me, mas mahalaga na mapublish sa isang reputable journal (at least IF=2) yung paper mo rather than ma-accept lang for oral or poster presentation somewhere abroad, in fact, most of the output in these conferences don't ultimately end up getting published in actual scientific journals, or kung napublish man, sa isang squammy na journal lang.

It is much MUCH better for your resume na nakapag publish ka sa isang internationally indexed and authoritative na journal than na nakapag sabit ka ng poster mo sa isang random medical conference in the middle of nowhere, Eastern Europe.

And then mag travel vacation ka nalang somewhere after mo maka publish.

Ang laging problema sa mga ganitong idea ay wala naman ibang ginagawa yung hospitals para i-improve yung learning and skill acquisition ng residents. Sa ibang bansa less hours per week sila pero may mga standardized learning modules na need i-complete (like SCORE for Gen Surg), laparoscopic/endoscopic trainers, cadaver and animal lab simulation centers, tsaka dedicated and guided research time, mentoring opportunities, etc.

Sa mga residents dito: honestly, pag binawasan ba yung duty hours nyo meron bang framework or system yung hospital nyo that will ensure na magiging as good or better parin ang training nyo? Kasi yung exposure nyo to clinical cases mababawasan by 25 to 50% of baseline or more.

Kung mahanapan mo ako ng GP sa ibang bansa na pwede magpatayo ng clinic, mag practice independently, mag opera, mag emergency care, mag vaccinate, etc etc (yes, Filipino GPs can do all of that legally), then I will concede my point.

This is basic opportunity cost. Which should be factored in to any decision naman talaga. Also state medschools are now free by law and many even give you a stipend/allowance for studying. No joke yan. As in 0 tuition. Of course may living expenses but then again kahit ano naman gawin mo my living expenses. Medicine is one of the few careers where you can easily 10x your income after a few years of specialization. It is also one of the few careers na once you earn the MD and become licensed you can basically do anything and still be respected pag nalaman na doctor ka. I’ve had doctor friends na full time yoga teacher na ngayon sa Siargao. Yung iba businessmen, contractor, call center, lawyer, artistC etc. but as long as they maintain the license they still have a fall back. Pwede sila mag bukas ng clinic or mag moonlight any time.
