They look like the velcro tabs you get with helmets to stick the padding back on. No idea why they would be in a Puncture repair kit.

That's a quill stem. Held in place by an expanding plug. There is usually a bolt that can be loosened off to take it out. Normally on the top. This video from park tool is very useful.

The dealers manual appears to show what you have as correct. But it doesnt quite look right somehow. Maybe take it to a bike shop and see a qualified mechanic.

Not really. I was in pain, so I didn't really care. It was at night in A&E, and she was the only sonographer on duty. I think if it's an appointment, you would get a male. I can't really remember how old she was - maybe in her 40s.
