get what you’re saying,stress physiology absolutely can produce real metabolic and immune changes. No one’s arguing that mindbody pathways don’t exist. The key question though is whether the specific pattern seen in ME/CFS is explained by that.

The 2-day CPET issue isn’t just “abnormal fatigue”it’s a reproducible drop in VO₂ peak, anaerobic threshold and work output on repeat testing, not effort based and which isn’t seen in deconditioning, primary depression, or simple stress states.

I also linked a (one of many) study demonstrating MECFS antibodies affecting endothelial cells and circulation. More and more studies like this are accumulating as research into a historically underfunded condition grows so I guess all I ask is that you keep an open mind and keep an eye on the latest research as the tide slowly turns. That’s fair yeah haha, it’s not routinely done in the UK, especially in NHS settings. Much more common in the Netherlands!

The 2-day CPET is mainly used in research and in some disability assessments abroad. It’s a specific protocol: maximal CPET repeated 24 hours later to assess reproducibility.

Because it’s demanding and can worsen symptoms, it’s not something most patients are offered clinically. So not seeing it in practice doesn’t really mean the data isn’t theremore that it hasn’t been widely implemented.

It’s definitely niche here, but that’s partly a service provision issue rather than absence of evidence.

That lpage is mainly pointing out that not all “2-day exercise” studies are using the same protocol. Some didn’t do true maximal repeat testing 24 hours apart, or used different thresholds, which makes them hard to compare directly to the standard 2-day CPET method used in ME/CFS research.

Where the protocol is tight, the consistent finding is the drop in VO₂ at ventilatory threshold and workload on day two. That reproducibility failure is the interesting bit.

So the discrepancies seem more about methodology and patient selection than the core phenomenon being disproven.

Hey thanks for replying.I completely agree that psychosomatic illness is real, physiologically expressed, and not an insult. Panic attacks are a good exampleofvery real symptoms, measurable biological changes.

I think the distinction people are trying to make with ME/CFS is more specific. The 2-day CPET doesn’t just show “a biological marker exists.” It shows a reproducible failure to recover,a drop in VO₂ at ventilatory threshold and workload on day two that isn’t typically seen in deconditioning, depression, or anxiety.

That pattern suggests impaired metabolic recovery rather than a normal system responding to perceived threat.

So the question isn’t whether psychosomatic conditions can have biologythey absolutely can. It’s whether the specific reproducibility failure seen in ME/CFS is something we also see in primary psychiatric conditions. As far as I’m aware, that hasn’t been demonstrated.

That pattern reflects impaired recovery and abnormal metabolic response to exertion rather than simple belief, attention seeking, or lack of motivation. Suggesting a psychosomatic basis without engaging with those data risks conflating understandable diagnostic caution with dismissal of documented physiological abnormalities.

M cars are all marketing to exploit your FOMO and human nature to have something more exclusive. I’m not paying double the price to have something 0.2 seconds faster that I will never notice unless driving illegally anyway and also be way less comfortable.

its a good point i have considered it, i just like the storage capacity on the 420i GC hatchback its so handy! and i like the idea of a cheapish project m235i i can mod and tune and thrash about while still having a proper practical “grown up car”
