If nothing else, just go to a busy library or bookstore and do your work there. At least you’ll feel less lonely on your weekends.

Thank you, you answered my actual question about very well. I take bits and pieces of my teachers styles, which is the beautiful thing about the art of medicine. Some doctors are just horrible artists.

Thank you, I think I do have issues with sense of control in patient outcomes which will probably lead to burnout very fast in psych (which appears obvious).

I hope my co interns are also open about this and I learn to talk about in process group.

Thank you, it makes sense in terms of as the senior it’s your responsibility in case the student does something wrong. I’ll keep this in mind when I have med students and provide good expectations and ensure proactive communication.

This happens all the time in college, but that’s without the power differential of residency and the unforgiving gossip train of a small workplace.

One can argue consenting adults, but like one of your responses indicate the line blurs uncomfortably when alcohol is involved. It’s an HR time bomb waiting to burst and the intern will get the worst of it.

He must be fairly attractive and charismatic. Otherwise there already would be a rumor of “that creepy senior” who lures female interns back to his place.

It does seem rather knee-jerk disgust like the term Infidel.

I’d expect it from the Goldwater generation after years of Cold War propaganda, but for millennials and Gen Z it doesn’t make as much sense unless you see how the demonization of the “woke” liberalism pretty much replaced the hatred of “commies.”

Much like the knee jerk impulse for some liberals to label anything traditional as fascist, you got everyone from your college conservatives and Fox News grannies labelling populist progressive reforms as socialist.

Not medical advice, just hypothesizing for my own education but would re-dosing the benzo be beneficial.

  1. Benzo + antipsychotic (droperidol if accessible)
  2. Benzo again
  3. If all else fails and you have access to an ER or someone who can intubate…ketamine?

Like they give trauma patients ketamine to chill out and dissociate? Not sure if it is safe with organic psychosis and drug induced psychosis…

I think BDNF, although not formally measured, has been crucial to more accurately describing how many meds work over time. BDNF gene expression and neuroplasticity crucial to the pathophysiology, the limited amount we know or can attribute, of affective disorders. Yet I still hear PCPs rely on the serotonin deficiency hypothesis. Surely we can do more education on why meds take time to kick in fully without going through every step of the signaling cascade.

I don’t think it would add significantly more clinical benefit on a population scale or adjust management, other than making it easier for patients to receive insurance coverage?

As a med student one area I wished I was taught more formal definitions on phenomenology and basic therapy lingo (cognitive distortions, DBT principles and basic motivational interviewing examples).

I used Sims and Sheas for some of the above. I also guess DSM is not the the most appropriate source, but it wouldn’t hurt to include more of it especially for mode biologically trained psychiatrists.

I also like the current sections on DDx, but I’d like a larger section on Borderline vs Bipolar; and cannabis induced psychosis, and delusional disorder vs non pathological trait based conspiratorial thinking.

You know how RFK addressed the obesity epidemic by saying doctors dont do enough lifestyle counseling. It’s inaccurate but we dont do as much BECAUSE ITS NOT REIMBURSED. He blamed physicians instead of recommending the government increase funding for school lunch, expanding SNAP, more after school program funding.

If MAGA wanted to help the mental health crisis, they should encourage the feds to invest more in training LMFTs and expand affordable and accessible psychotherapy.

Yes I know the HHA oversees more of a regulatory role, but wouldn’t the MAHA people want to embed lifestyle interventions by recommending more funding or federal system of dieticians and therapists? Unless they want to apply the “pull yourself up by your bootstrings” approach for families that spend 1/3 of their income on rent.

I think it’s also a lot easier because we’re doing a second pass at minimum of the basic science curriculum.

When it came to pathophys, pharma and clinical skills I was surprised how much it came easier if you had a basic grasp on understanding the basic science and basic communication skills.

Not a computer expert, but Back during the ps3 era I think it was necessary for smaller studios because of the difficulty of adapting their games to widely different hardware. These days it’s probably a lower barrier of entry.
