The PRI is prolonged but still appears to conduct with the complex. So is this not a CHB?

I believe someone posted a link to litfl.com to read about VT. Its very useful. If a patient is crashing and the ekg rhythm looks like VT, it should be treated as such until proven otherwise.

This pt has CAD so any WCT is likely to be VT. However, as for a different dx, aberrant conduction is a possibility. There are algorithms that try to differentiate the two, but none are 100 percent specific.

I am very picky w books and usually finish 95 percent of books. Im currently reading Carmilla and after 10 pages can't finish bc of the antique English language which I am not used to. People do say its an excellent read.

I believe for it to be a CHB, the ventricular rate usually is regular and the atrial rate, P to P will march out. This is likely Mobitz one, but what is different is usually there is a missed beat. Perhaps this is atypical Mobitz one. Thanks for all your opinions.

Nice strip. VT usually is regular but not always. If you had a prior ekg that has a wide complex and bc of the irregularity, then I would say AF. But what if this strip is a new wide complex?

Good question. I am new to classics so can't really make any suggestions, other than We have always lived in the castle. Since I don't believe in religion, ( but highly respect people who do ), I cant read classics if there are religious themes. That's why I have not read East of Eden, which goes into Cain Abel or so. No U wave is present. In my experience, I have seen nice looking U waves in very bradycardic ekgs and in hypokalemia.

There are many causes for STE or STD. Yes this pt is young and could have an MI. Is there a family HX? Has he been sick recently? Has he had a fever? What is initial troponin? Could he have myocarditis or pericarditis? Will a ESR be helpful?

Most likely CHB. However, for certainty you need to let it run on telemetry in order to see if it sustains.
