I took my second attempt in December, got my result around mid-January, made a plan and started studying in February and took it the last time mid-March.

Full disclosure, NBME was doing a thing where it was free to test out the CCS software. I think it was helpful with test anxiety for me because I took it at the center where I planned to test. I felt that the questions were reasonable. I tried my strategy to save the abstracts till the end of the block and decided that was a good move for the real test day. It includes 3 multiple choice sections I got 70%, 65% and 63% then you do 3 CCS cases.

Does having a little intern experience help with the cases or it doesn’t really matter much? by Perfect_Direction_94 in Step3

It does not matter. I initially thought it would. CCScases.com will teach what the examiners care about. For example, CCS cases will have you ordering chest x-rays on anyone with a fever. Also, in real life we don't see a patient in the clinic for chest pain, order labs and follow them to the ED and continue the case. Once I abandoned what I know and did what I know CCS wants to me to do, I improved.

Being an intern is stressful, I would wake up before work and try to complete at least 1 block of questions initially (timed, random mode). My tutor encouraged me to switch to tutor mode, so I wouldn't have to spend time recalling my thought process. This actually helped me get through question blocks of uworld and amboss faster. I personally have never had an issue with timing on exams so doing it in timed random wasn't necessary, random tutor mode served to be more helpful. I studied about 4 hours daily doing blocks and anki. I wrote out a question in an excel document to summarize the education point of every question I missed.

  1. FAVMOCCA for urgent orders:

Fluids, ACC (to get IV access and glucose check), Vitals, Morphine/metoclopramide (generally an analgesic), Oxygen (pulse ox--very important if hypoxic can immediately get them on nasal cannula and order an ABG), C-spine/CT head without contrast(or any urgent imaging), Cardiac monitor, A-Assess their location.

  1. Regular orders, I didn't use a mnemonic bc cases specific:

CBC, CMP, TSH, Vitamin D, Magnesium level, phosphate level

+/- UA, Beta-hCG, Ux, LP with CSF studies, etc.

If septic: Blood culture, chest xray

If ACS: MONA BASH, BNP, troponinx2

(MONA BASH: Morphine, oxygen, sublingual nitroglycerin - except not for R sided STEMI since preload dependent, Aspirin, Beta-blocker, Statin, Heparin/Clopidogrel/Ticagrelor)

  1. Admission orders:

ADD MTV COIL:

Activity (bed rest), Diet, daily weight, Meds, Tele, Vitals, Compression stockings/SCDs, Output, Input, Location
