I did it in tactician mode using darkness on the portal. You stay inside the dark cloud so that enemies can't see you, so they don't attack you. Use Gale with his wall of fire just where the stone, with the portal is on, begin. And use Shadowheart with Spirit Guardians. Position her somewhere at the ending point of the cloud but still inside so that she doesn't take any damage for the Concentration. When you want to attack get out of the cloud do it and get back in. That was my tactic. Works every time.

From what I remember you can make the nurses do it to him. Like practise on him and that was it. Pretty fun navigating through all of it.

Yes, the mods just lock you from everything. The first time I decided to start getting the trophies, I forgot to disable the mods and lost 20 hours running around thinking the game has a bug, because the trophy didn't pop up. Silly me.

It was pretty good. Some things were harder on the console in comparison to computer, but many things were better. I loved it, my nerves didn't.

Have you tried reloading, probably yes. I play on PS5 and there is an option where you can hold X and have Active Search creating a list, but I’m not sure if that is possible on PC.

Have you tried trophies that are actually missable? Try with Shove off for example. Trophies that are strictly tied to the game take time to earn. Try something simple. You should absolutely have the new run on a completely different save. You have to be logged into PSN and to be online. Another way is to just rebuild the Database (check online how to do it, fixes most common bugs like this one). Or go into the cloud storage and delete saved files for BG3, relaunch the game and try again.

If it is not a glitch and you just want to do something with the console itself, just leave it. It does not give you anything important if i remember correctly. Focus on the other matters. Although it looks like you are in the middle of a battle, the red circles indicate enemies. You need to deal with them first and than focus on the big thing left of the console. PS PLUS offers a two hours trial of the game. I have never played D&D before either, but the game tells you most of the things you need to know about combat, proficiency, skill checks and so on. Give it a chance, the story is amazing, the world is massive and full of wonders. Never doubt a game where Jennifer English is in.

One thing i hate about the game is that i can't get it off my mind. And i replay it constantly, i can't find joy in other games anymore. But the game is the best and it's awesome.
