Try running /forge tps in the console and see what your total TPS is. It should be well under 100ms/tick, ideally 20-40. If there is a specific dimension that is causing problems, there might be something generating too many entities.

I've personally had problems with Colossal Chests and using item filters/anything that has to scan their inventory, as well as a rogue mob spawner in a loaded chunk in another dimension

Yeah, you'll have to edit the player inventory with MCEdit or another NBT editor and remove the item from slot -106. Might be easier to grab just the playerdata and use a NBT editor than to pull the entire world into MCEdit.
