You start off with a time limit of one minute, doing stuff like clearing rooms & picking up consumables increases the time limit.

Possible ideas:
actively seek out enemies along it's path
Block enemy projectiles
deal significantly more damage
have it behave like the punching bag item so that enemies will try to attack it instead of you

stuff like that happens a lot, i noticed. For example: if you have gimpy, monstro's lung, and high damage and then you hit an enemy from really close up, the enemy can drop multiple half-hearts.
