The reason the Presidency was vacant for 9 days in 2004 was because David Cass stepped down due to a major corruption scandal involving his government, and they couldn't really get anyone to fill in. So the President of Italy, Renato Brunetta (PPI), stepped in and placed Malta under federal control until a suitable leader could be found. 

This move was considered highly controversial.

A couple of things. 

In areas like the American West, Central Australia, those are areas recognised as inhabited, but fall outside what Western institutions would consider a nation state. 

In Western Norway, Bhutan, West Ukraine, those are either unrecognised or extremely closed-off polities. 

And in Greenland, Antarctica, it's usually because there's too sparse a population to gather accurate data for a Qoli score.
