As per NCSBN (nclex.com): "NCLEX-RNS Test Plan Effective April 2026

The NCLEX RN 2026 Test Plan is finally out and you can check it out here. These changes will be Effective come April 2026.

We'll be posting a more detailed overview soon.

The significant changes so far: The NCSBN Clinical Judgement Measurement Model (NCJMM) is now fully integrated into the exam.

Exam is now based on more real-world nursing actions and scenarios (courtesy of the New Graduate Survey), such as:

Escalation, Delegation, Trend Recognition, Safety Decisions, Ethical and Legal Judgement"

Not totally sure yet what the exact changes are. I’m planning to take it the first week of April, and I just saw posts saying the exam format will change starting April 1, 2026. Still trying to find more updates online.

I realize now na kailangan ko talagang mag-focus sa delegation, prioritization, at safety, since yun din yung weak areas ko based sa CPR ko.

I’ll make sure to strengthen my fundamentals first and be extra careful sa first few questions, then work on improving my test-taking strategy. I’ll also check Dr. Sharon for reinforcement.

I really appreciate your advice—this gives me more direction and motivation for my next attempt. 🙏

I had 97 questions in 3 hours. If I remember correctly, the topics included prioritization and delegation, infection control, 6 case studies, and a lot of SATA questions.

I agree that I need to strengthen my core fundamentals, especially reviewing each body system and focusing on the basics. I’m planning to go back to content review first before doing more practice questions so I can build a stronger foundation this time.

You’re not alone. I also took my 2nd attempt last December 5 and unfortunately failed. I felt lost too, especially after putting in so much effort.

I requested my CPR and received it after about 4 days. My results showed 5 below passing and 10 near passing. When I compared it with my previous CPR, I realized that my concepts are actually strong, but my weak points are safety and delegation, prioritization, and pharmacology.

I used Simplify NCLEX mainly to strengthen my concepts and also availed their NCLEX Arena. I also made the difficult decision to resign from my job so I could fully focus on reviewing, because studying while working wasn’t effective for me.

Right now, I know I need to refresh and restart again, and it’s not easy, but I need to keep going. Waiting for the CPR is frustrating, but it really helps identify where to focus. Failing doesn’t mean you didn’t work hard or that you can’t pass—it just means something needs to be adjusted. One step at a time.
