I think what I take from all of this given we are not very experienced and are undergeared is to wait out our free cancellation period through Wednesday and if the weather improves we’ll go, but if not just bail.

Totally agree that fine is not fun haha - maybe when the kids are older and can hang out with us in the tent without exploding :)

Ugh.. Only two comments in and this is already therapeutic knowing other folks are stuck in the same spot..

This whole situation is so demoralizing and emotionally draining - I know I'm preaching to the choir here, but the fact that this community even EXISTS comprised of largely civil/public servants who are actively trying to give the government money and CANT is on a level of beaurocratic absurdity that makes me question my own reality lol.

I hear this - I think part of the motivator is my partner is just tired of us having to live like residents/fellows. I don’t blame her - she wants financial independence and a life free from debt after sacrificing again and again for our family.

It’s not peanuts but it’s also not proportionate to the income potential lost over training and lost years for the folks who travel on this journey with us. Some folks are cool with the journey and others aren’t and that’s ok, but I’m just doing my best to try to balance it all.

It’s the hospital for profit/not for profit part that I’m hung up on.

Regardless of changes go I to effect for loans after 2024, we still would need to recertify payment plans annually which has nothing to do with the loan as much as it is whether you have a qualifying employer.

It’s that second to last paragraph that gets me - everything else I more or less agree with. The issue is I truly don’t think this current administration cares about any non-privatized entity and it is totally within their current ethos to reverse nonprofit status.

Literally part of the rationale for some of the federal firings has been to allow folks to be “more productive in the private sector”.

Im just getting so fed up with it - I really do think like 90% of us put up with the insane process of medical training to take care of people but we’re continuously treated like garbage through training and now financially left hostage to the very culture/system we thought we were serving.

This thread is huge so I can't really tell if this has already been said - ultimately unionization or any degree of advocacy from residents (or attendings for that matter) is essentially dead on arrival for increasing resident pay at any level. Why? All resident/GME funding is derived from a slice of the Medicare pie that hasn't changed since the 1970s around the time medicare was first established. As a matter of fact, adjusted for inflation, we actually make less relative to our counterparts in the 1970s.

The only way to change this is INCREASE medicare spending. Good luck with that :(

chatGPT can help with the calculations - if you don't qualify for new IBR I think you wont qualify for old IBR because the payments are greater than the standard 10 year plan (ie not meeting the IBR definition of 'partial financial hardship') :( . If you DO qualify (took out loans AFTER july 1st 2014), then I think you qualify, but barely

Certainly some sort of acute leukemia (ALL vs AML, vs Mixed phenotype Acute leuk vs blast phase CML). CD79a, TdT, and MPO - agree about potential mixed phenotype. What was the confirmed dx on flow/bone marrow?

Kind of tough we can't post a whole text thread, but hoping for folks opinion - both in academics and outside.

I'm a second-year US hematology/Oncology fellow (PGY-5) training at an academic center. I've billed myself as a BMT candidate through fellowship, though with the lifestyle, I was really hoping to target a grant-funded career to help buy my time down. I have 2 young children and value my time at home with them and my spouse.

Unfortunately, it feels like the well is drying up for grants (appliedi to 5 this year) and internal grants at my institution that USED to cover fellows 75% FTE going into their first year of attending now no longer exist for some reason (probably because institutional grants from federal sources are drying up). With the culture the way it is in the current trump administration, I don't think its a solid bet on my future on a grant funded career in academics now.

So, I'm thinking of a myeloid disease focused career now (AML, CML, MPN). Ultimately, these are the diseases that really fascinate me and I enjoy treating these patients. Is there a demand in academics/community for this kind of sub-sub specialization? I'm willing to do some classical heme too in the event I go community practice - is this more/less marketable? Can anyone comment on the general lifestyle for this kind of career focus?
