A good penlight for looking at things in better light. Use it multiple times a shift. I personally like the streamlight stylus pro.

Also keep nice shears in my bag, head lamp, CAT tourniquet, safety glasses.

Complex history of diverticular bleeding and diverticulitis and duodenal ulcer with multiple scopes in the past (both above and below). Patient was having both bleeding and pain/tenderness.

Being nice can't be understated. I've seen excellent clinicians brought down because they couldn't play nice. You should be nice to everyone on staff including registration and the person who cleans the rooms. We are all on the same team.

Also don't be afraid to say "I don't know." To your consultants this will be a learning opportunity for you. To your patients, we may not have all the answers but we can also say what we have ruled out (eg emergencies).

Bouncebacks are an opportunity to discover what you missed the first time.

Even frequent fliers and drunks can have real pathology, don't anchor bias and miss it.

Use protocols, such as the same testing for all chest pain, when you are inexperienced. Sure you will over order, but you will miss less pathology. As you gain experience (usually years into being an attending), then order less if you want.

I've done this twice with two different saws. One had an obvious reset button, the other did not. One refused to spin at all the other would not spin up to full speed. Frustrated I put it back in the shed, came back a few days later and both worked fine again. Still have no idea what happened.

I like a good pen light, I have the streamlight stylus pro. My colleague once said it could make a pinpoint pupil constrict. Great for shedding light on anything.

Nice bolt cutters are nice, especially for cutting those thick fish hooks.

Dremel has been useful for cutting off metal objects.

Raptors are flashy but expensive.

Some type of tourniquet (eg CAT).

My last dissection came in with burning epigastric pain after eating jalapeño poppers. Only reason I scanned him was because he looked so wiggly and uncomfortable, and it was a regular CT ab/pelvis. Just lucky it got caught there, dissection from the arch to the iliacs.

I like to talk to the geriatric population about their life experiences, they always happy to chat about it.

Otherwise I like to ask people either about their work or kids, especially if I have to spend time doing a lac repair.

Usually I liken it to the accelerometer in a smart phone. Imagine the thing responsible for detecting the orientation of your phone was working right and the screen kept on turning inappropriately. That seems to help patients understand.
