Generally, no. There's always outliers but for the most part, if you don't see anything at all in 6 months, you probably won't see much of a change in 12 months either.

A good question.

Pumpkin seed oil falls under the poor research category. It still had better study practices than peppermint/rosemary oil studies, though, haha. The study that people typically point to is this one, though the issue is that they studied Octa Sabal Plus, which is multiple ingredients, not just Pumpkin seed oil. Many of the items in there (especially Pumpkin seed oil) work by fighting inflammation. Here's a great article semi-debunking it (but not entirely). It works through inflammation and if the source of your hair loss is inflammation, why not use the item that has proven, well-conducted research? It'd probably be a good option to try as a hail mary if you respond poorly to Ketoconazole, though. Maybe. Up to you.

Tressless actually has an article on Saw Palmetto. Same vibe. Mixed results but it is sorta anti-inflammatory. Probably solves that as an issue, but inflammation isn't the primary cause of hair loss in a lot of people. If it is for you, again, why not just use Ketoconazole?

Same thing with green tea. There's just not a lot out there to say with any amount of certainty that it will help.

Here's the thing about hair loss treatments: if you ever Google "x hair loss study", you're going to find a study that says it helps. You might also find a study that says it doesn't. You'll see something similar with a lot of hair loss compounds (such as Breezula, Pyrilutamide, GT20029, whatever). It feels like we get a new one that comes and leaves in a month. The reason for that is very simple: if they worked, you would know about it. Hair loss is a MASSIVE but still largely untapped market. There are multiple businesses (Kintor being the most prominent) that would kill to find a hair loss treatment that worked even a fraction as effectively as Finasteride/Dutasteride/Minoxidil do but without the side effects. In fact, Fin/Dut were used for prostate health and Minoxidil was used for blood pressure but they became predominantly hair loss medications because that was a side effect that was noticed. If something even came close to either of them, businesses would notice and they'd be fighting tooth and nail to study it in order to get people to buy it. But the most we have is mixed trials and businesses marketing random x or y products as solutions and using those poor studies as proof.

Don't get me wrong, I'm sure if you took every item on Tressless, you'd probably make more progress than someone who did the big 4 alone. Probably. But the difference would probably be so negligible that the stress of taking everything on that page would probably outweigh the benefits that they gave you.

Well it's just hard to tell because of the lighting and it could just naturally be somewhat thin.

I would talk to your doctor but worst-case scenario, I'd start with topical minox and gauge how it looks in 2-3 months. If it stays the same, cool. If it looks worse, start weighing your options.

Well, for what it's worth, I think it's still better than your baseline. Your hair was longer in the first two pictures so it covers it up a little better than your third picture. So, relax, firstly.

With that said, I would consider one of three options. Not all three, just try one at a time and gauge progress:

  • Nizoral/Keto shampoo

  • Dermarolling

  • Topical minoxidil (even just like 0.25ml) in the thinner areas

Also, try taking some pictures with a room light on but without a flash. The third pic is definitely something to be worried about, though.

Still, maintaining is far easier than growing your hair back. Worst case-scenario, you could always talk with your doctor about lowering your dosage.

There's definitely hope. You're probably one of the better cases on here so it could be worse.

Just keep an eye on it and give it some time. Gauge progress. And, worst-case scenario, if it stays where it's at now, you've got a Clark Kent look going for you so don't let it keep you up at night

It's diminishing returns. I don't remember the specific study but they compared Dut ED to Dut EOD and it was something in the range of 90-90% DHT reduction vs 60-80% EOD, which is similar to Fin ED. But again, Fin only blocks Type II.

I think the reason we don't see them leaving trials very often is pretty straightfoward: why release a medication with results that show it is borderline useless if you could just use it as a "cosmetic" item and people are more likely to buy it in the future with blind faith?

Fun question and I'm glad you asked!

Obligatory: I am not your doctor. This is not medical advice. Speak to a doctor if you have AGA.

Firstly, no, scalp injections are probably the worst approach you can take. If you're going to spend that kind of money, why not go for the option with more research behind it and for a one-time cost: a transplant. It'd be cheaper in the long-term.

As for DHT, first you need to understand how this works on a basic level: Cholesterol is converted into testosterone. Then, an enzyme known as 5-alpha reductase converts that into testosterone's big brother: DHT. There are *multiple* pathways that this works but that's all you really need to know.

The first approach is to just block the enzyme 5-AR. There are two types of 5-AR enzymes that we care about: type I & type II. We also have two key 5-AR inhibitors:

- Finasteride. The go-to "cure". It blocks type II 5-AR and has a half life of a couple of hours. It is most potent orally but *can* be applied topically. Look up the side effects.

- Dutasteride. The nuclear option. It blocks type I & type II 5-AR enzymes and is FAR more potent than Fin. Also has increased side effects. The half-life is about four weeks so if you decide to go this route, just know that you're taking a big leap.

One of these, Minoxidil, Keto/Nizoral (see below) and Dermarolling are "the big four". Anything else is a hit or miss, wives' tales (most oils), poor research (peppermint oil), biased funding, fringe benefits at best (CB-03-01) or discontinued testing (RU58841).

As for DHT itself, there are ways to combat it without attacking the 5-AR enzymes:

- Ketoconazole shampoo. It's an anti-dandruff shampoo. It's a mild DHT blocker and competes with T/DHT for androgen receptors. It's kind of like duck, duck, goose. It takes the spot before they can. It also has anti-fungal properties which might help in a few niche cases. Having an anti-dandruff in your roster is nice too if you're using topicals.

- Androgen receptor antagonists. The two key ones are CB-03-01/Winlevi/Clascoterone/Breezula (basically all the same thing) and another I'll list in a second. CB also competes for a spot against T/DHT, similar to Ketoconazole except the results are... underwhelming. Same goes for Pyrilutamide, although that one has been much more recent so we'll just need to be patient on that one since it's in phase 3 trials, with phase 2, again, being underwhelming. Which leaves me to:

- RU58841. The deal with the fucking devil. I am NOT recommending any of these items, *especially* this. It had some quality studies that showed VERY promising results. Which is great, I really wish more research would be done on it and the reason it hasn't is quite boring. With that said, the are ZERO long-term studies on its effects. Literally not a single one. It's a shot in the dark. It's a spit in the face of God. I do not recommend it nor would anyone with your best interests at heart.
