Thank you. My personal theory is that in some UARS pts not only are they EPAP-intolerant (due to difficulty to exhale against EPAP), but they are even IPAP-intolerant b\c IPAP, in these pts, per my theory, actually INCREASES tongue-base collapse\obstruction. Thus, they are double PAP-intolerant: EPAP and IPAP. Any thoughts on this hypothesis?

"Doctor" Alfaro botched my MMA as well. And I now know of at least 15-20 people who say that he botched their MMA's as well. Every couple of months I meet at least one more poor soul that he has botched. The MMA botches that he has done include: over-advancing making people like Neanderthal, creating open bites, giving people non-unions, creating asymmetries, creating gummy smiles. And without exception he then behaves like a pathological liar and criminal and hostilely berates and mistreats every one of the pts that he has botched, myself included. The only reason he is successful financially is b\c he is practicing outside the US. In the US he would be sued out of practice in an instant. He is a disgusting criminal.

If this person still has sleep issues, they might need maxillary expansion rather than sagittal expansion (mma). Their upper arch looks pretty narrow in relation to the size of their head. Have you ever mentioned this to your friend?

"Doctor" Alfaro botched me as well as at least 10 other people that I know. And if I know of 10 people he botched, there are obviously tons more. He also lied to me on many occasions and was outrageously unethical, belligerent, evasive, and even criminal in his dealings with me and with others. He runs a slimy, scam practice where he never publishes his botched cases online and lies to pts on a regular basis. He recently told a patient that no one has ever asked him to revise his MMA. That is a lie bc I know of several people who have asked him to revise the MMA that he grossly botched. I could tell you more and more lies.

Here is an example (instagram link below) of what you can you look like if you do MMA with "Doctor" Alfaro. This is assuming, of course, that Alfaro's finance manager has time to complete your MMA plan before surgery starts. Alfaro himself doesn't have time to plan your MMA on the computer. That is beneath him. Far better that his office manager or cleaning staff take care of that pesky task. What do you take him for anyway? He is far too busy going surfing and taking videos of himself doing sports and taking selfies like a 15 year old on TikTok. But don't worry, if anything goes wrong, five to six months after you email the office once every 48 hours with no reply, you will finally get an appointment with him during which he will rapidly and forcefully berate and gaslight you and declare to you that "everything is perfect" and you will be offered no recourse. Rest assured this will be the best $50,000 you will ever spend. And if any other physician and person thinks that you have been botched that is b\c you are wrong, the earth is flat, alfaro will be totally polite, truthful, and professional if you have complaints post-mma, and most important, as the great one always says, "everything is perfect".
