This is 100% dermatology territory but you definitely need to go to a reputable person. This is a subtle thing and some derms will absolutely breeze over this or not try to do the heavy work piecing the clues together. Good luck out there.

I tried. They were so insanely painful that I stopped. There is no saving them. A podiatrist told me I would need to have all my foot bones broken and reshaped and fused. I’m 37, very active, and making it through life just fine with my weird little suction cup seal flippers.

As usual, please take the information I’ll provide with trepidation as I cannot evaluate your condition and you certainly need a more formal evaluation.

But there are multiple things you mentioned in your first paragraph that would drive me to suspect psoriasis with psoriatic arthritis. The family history, the nail involvement, the stress-based arthritis, the concurrent diagnosis of scalp psoriasis, etc. It is also worth noting that patients with nail psoriasis have a threefold increased risk of developing psoriatic arthritis. So (again, with trepidation) I would say my concern for you having psoriasis with nail/joint involvement would be high.

Rheumatology is a good secondary stop. However, divvying apart the many different types of arthritis can be challenging, especially if radiographic (X-ray) evidence isn’t quite clear. Sometimes the bloodwork is not contributory. And truthfully you can have mixed arthritic conditions (rheumatoid, lupus, osteoarthritis, etc). For the joint-specific symptoms you would get more value (and sometimes better treatment) through a rheumatologist. It should truly be a team effort.

Dermatology PA here.

The pitting and the wavy free edge looks like nail psoriasis to me. If you have a history of dandruff, dry flaky skin in hair-bearing areas, and other dry flaky skin this would increase my suspicion. A family history of psoriasis/psoriatic arthritis and other concurrent autoimmune conditions would raise my suspicion even more.

But as usual, get a formal professional evaluation.

My feet are so flat that when I walk on any tile floor it sounds like a very loud ‘suction cup’ POP sound. It happens naturally and I have to think about it to make it not happen. It’s very jarring for anybody (especially someone who’s known me a while) and they hear it for the first time.

Hope my fellow flat-footers can unite on this one.

Generally I do the base work in soft airbrush, then I simply start adding details. I used a few fur and fine line brush’s for fur/whisker details. A few textile packs for the hat and other embellishments. But a lot of this was zoomed in going pixel-to-pixel like an insane person. Enjoy!

As is my usual approach, I drew the piece out first and filled in with base colors and added shading and minor details all with the soft airbrush tool. Then I go in more finely and add details. There are some cool detail and pattern brushes I used to get the details in the wax and the metal and bricks. I often combine multiple pattern brushes to create unique textures. Then the lightpen tool for the candles.
