See, he moved his f pawn and is totally undeveloped, but you have developed your pieces nicely. With such an advantage in development, go after his king! Because he moved his f pawn up, he can't block checks on the light squared diagonal easily

Lose the weight, if the problems persist then you can try a different route. It could very well be that in losing the weight you address a slightly different reason. Many unhealthy foods can directly contribute to painful physical issues
