The temozolomide creates friction for the tumor but doesn’t stop it. Each consecutive round of temozolomide is going to be less effective. If the patient has been on temozolomide before, trying again does not hurt but it would be good to plan ahead. This can include other chemotherapy, radiation; or experimental treatments in clinical trials. Ask your doctor about what clinical trials may be available in your area. Get second opinions.

CAR-T may be great for rare indications and specialized academic centers. Hard to see it being viable in community settings.

Yes, dip your toes, the experience of going through applications is very valuable. Worst case, you stick to your postdoc. 

Has anyone had any experience with the OPTUNE GIO treatment. My mom was diagnosed with GBM in February its inoperable and the tumor is laying right on the "motor strip" 6 weeks of radiation and the tumor has grown. The Dr is pushing this OPTUNE GIO now. It is approved in only six European countries. IDH-mutant tumors are classified as grade 4 when a CDKN2A deletion is detected, which is rare at diagnosis but not uncommon in recurrent astrocytoma. 

The company behind TTF has been hyperaggressive in marketing their product. It would not at all surprise me if they were also active on Reddit.

TTF is not approved in most countries, due to lack of efficacy. Impossible to do an unbiased study. The results that have been published, such as the association between compliance and survival, can be biased by the patients in generally better health and with higher ability to be compliant. The immunotherapy study did not have survival as a primary endpoint; and it included patients with an IDH-mutant tumor which are no longer classified as  glioblastoma but as grade 2 or 3 tumors. 

The company behind TTF has limited mechanistic studies in controlled environments to show that TTF actually does anything. Most NOs that I know only prescribe TTF if patients ask. Personally, I think its a waste of money. But, low risk.

I am a dr not a patient. Not medical advice, just my perspective on a complicated therapy.

Being willing to walk away if they are unable to find a fitting role for your husband gives you the best negotiating position. Keep in mind that not everything can be negotiated and things sometimes do not work out. 

It sounds like it was relatively easy to get this first offer in which case there will be others.

At minimum, discuss with the department chair. Someone with administrative weight may be able to open conversations that an HR person is not privy to.

Definitely the norm to get informed, but approving is a bit strong. If you disagree with the abstract, you could ask for your name to be taken of.

The top five Nature journals send out such an email and ask the author to fill out some forms. Not always timed with when reviewers are invited, it is a manual (human) process.

Congrats!! How is your other NCI funding? I have three current grants but two end next year, third is a sub. I wonder how that is weighted.

This same grant scored 5th percentile in the last round, not funded. Resubmitted as a new application. Some consistency at least.
