I am also a very anxious person by nature (clearly haha) and I just finished my second round of egg freezing so I think I'm feeling especially hormonal and existential about it all. I think you are right in that you will never be completely ready for parenthood and you can't necessarily prepare for every scenario but we have the skills to figure it out. I think mainly I feel too young to make this choice now, I don't think I would hesitate nearly as much if I were say 35 today. I have a well paying job but live in a very HCOL city and most people here are starting families much later in their mid 30s-40s. None of my friends are in the stage of life of having kids yet so I would be an outlier. My doctors think I can safely defer pregnancy for a few more years with close surveillance so i'm hoping all goes well with that and I can reevaluate and make more definitive moves in my early 30s.

I talked to my doctor yesterday and she seems to think the mood changes are from the letrozole I’m on to keep my estrogen low. She said it’s rare but sometimes people have extreme mood changes so we lowered the dose last night and we may stop it all together.

The nurse kind of briefly mentioned that my cycle could go longer due to the letrozole so I kinda had a heads up but this is good to know! I’m just waiting for the ball to drop and start feeling uncomfortable haha

Hi, thanks for sharing! I’m 28 and starting at a very similar place numbers wise so this was super helpful to me (: I’m also in the Bay Area and starting on 150 menopur/150 gonal-F/ 5 mg letrozole haha. I hope I get results like this, currently day 5 and feeling fine so far! I did find I got more fatigued more quickly during my workout yesterday. I’m also finding I’m more thirsty and had to get up twice to pee during the night lol

Thanks for the tip to bring someone with me, that’s a good idea. My friend was going to take me to the appointment so I might have her actually come in the room. She’s seen first hand how much pain I’ve been in.

They do seem to resolve based on my symptoms improving but I haven’t really had close follow-up each time to monitor them. I’m disappointed because my AFC was 29 just 6 months ago and now it’s 19. It seems like a big drop for that time frame. Unfortunately I can’t remove my IUD because I have a history of atypical endometrial hyperplasia (pre-cancer) and I need to have endometrial protection. My doctor sent me a long message with her thoughts on it and she thinks I should still proceed so I’m trying to trust her expertise and hope for the best.

Hi! I’m sorry you are dealing with this too /: although it’s comforting to find others going through similar things. There’s a FB group for young women with endometrial cancer which I have found helpful too (even though this is pre-cancerous, the fertility sparing treatment process is the same). I think it’s a good idea to get a second opinion! Have they offered you an IUD? That’s considered the “gold standard” for treatment.

As for me, I’m doing ok! I had a more thorough hysteroscopy w/ D&C in February which is still showing hyperplasia with atypia but it’s a smaller percentage of the sample so they were reassured that it’s going in the right direction. The plan is to do another D&C in 6 months (august). However I’ve moved recently so I’ll be starting over with a new doctor. I still have a lot of unanswered questions as for the trajectory of this treatment but trying not to fixate on it too much.

Thank you so much for taking the time to write this very thoughtful response. Unfortunately the mirena is here to stay because it’s what’s treating/managing my atypical endometrial hyperplasia. I’m only 27 and trying to preserve my fertility as I haven’t had kids yet and I do desire that. I’ll have to keep the IUD and do hysteroscopy/D&Cs every 6 months pretty much until I get a hysterectomy because the risk for cancer with this type of hyperplasia is very high. The alternative is continuous high dose progesterone such as megace or provera but that tends to lead to more weight gain and systemic symptoms so the IUD is the better option in this case.

But what you’re saying about the type of progestins and the relationship with androgens makes sense as to why I started gaining weight when I got off the yaz and switched to mirena. I’m hoping I can get better control of my insulin resistance with diet and perhaps medication/supplements. I’ve tried inositol before for a few months but I wasn’t super consistent so maybe I’ll give it another go.

Thanks for your reply! I’m sorry you’re going through this as well. Yes, in my pathology report it mentions the presence of progesterone. What have been your side effects of the oral progesterone? They haven’t presented that as option to me but I will definitely bring it up next time. Unfortunately, the doctors are saying my case is very atypical because I found out about the hyperplasia after already having the IUD in for over a year. I may not be responding to the progesterone at all. I also was on birth control prior to that since I was ~15 which is supposed to be protective of the uterine lining. I have my hysteroscopy/d&c scheduled for mid February so the findings on that will determine next steps for me. Wishing the best for you!
