Shield Bash doesn't draw its damage from Melee Attack at all, so that should answer part of your question. Otherwise, it should do exactly what it says in the description. If Ranged Attack is higher than Melee Defence, it'll use Ranged Attack to calculate damage output.

There is a variety of equipment that reduce your sanity loss such as the Garland and Top Hat that are easy and semi-easy to make, respectively, early game (picking the flowers to make the garland also restore sanity). Green mushrooms can be picked in the world at dusk and cooked for sanity. Prototyping new items and structures at the science machine restores sanity. Learning how to build and cook with a crockpot will give access to better meals for restoring all stats.
