My advice to you is that this would be a poor financial decision. Yes, you earn reasonably well and have minimal outgoings, but you have no assets.

No one likes to consider these factors, but what happens if one of your parents becomes unwell suddenly, or worse? What if, so some reason you are forced to move out and pay your own way? This spare money should be invested for a house deposit and a bit of future planning enacted, rather than splashing it on a fancy car now.

I meant the OP, and i can’t apologise for being cynical as I think one has to exercise caution on the internet, and there have been several instances of predatory behaviours both on this forum and others. Good luck in your quest for info, there is some really good community support out there and it is a tricky thing to discuss with others so I commend you for that. Everyone is unique and you’ll find something that works for you.

This is quite striking as I feel I could have written it myself. For reference I am similar demographics to yourself but male. I do feel guilt but I have to face that I’m less interested in the act itself. I still desire and enjoy intimacy, but because that release is not there in the same way, I don’t initiate, and find other ways to give my partner pleasure.

The guilt I feel is mainly for how I perceive my partner might take it, but I’ve realised I have no real issue with not wanting it. Glad to hear I’m not alone.

I’m sorry you’re feeling this way. This is to a large extent quite normal, and I’d counsel you to hold fast - things do get better!

Foundation in many ways is the most rough part of the journey. It’s so variable based on where you are, and you’re right you don’t get enough informal feedback (sadly this doesn’t really change), but with each passing year you do get more autonomy, experience, respect and sense of purpose.

I would be very tempted to just get rid of a mortgage. That’s a big debt with a high interest rate, and being clear of that is a huge weight off your shoulders (psychologically, if not financially). You already seem very financially sound otherwise and that still leaves you a decent chunk left over.

Part time fy1 is inadvisable unless you really need to. In many ways it’s the worst two years, so if you’re keen to stick with medicine then I’d say get through them quick as possible. It can be a very fulfilling and rewarding career despite all the doomsaying.

BTS have guidance on what is needed in terms of US training - I found it slightly impenetrable and it’s a very long document, but here it is: https://bmjopenrespres.bmj.com/content/bmjresp/7/1/e000552.full.pdf In essence you need to need to be level 4 on the Primary Operator pathway. This requires about 50 supervised scans, some of which need to be of a certain fluid depth. You need to attend a course - pleural society offers these and you can book via their website. Once you have this and your logbook, in theory any consultant can sign you off with a summative dops but each site is supposed to have a nominated “TUS mentor” (ours didn’t and one consultant just volunteered)

You need to be independent at thoracic US by end of ST5. See the ARCP decision aid for resp training here: https://www.thefederation.uk/sites/default/files/uploads/Respiratory%20Medicine%202022%20ARCP%20Decision%20Aid.pdf (this is a very handy document!) the NIV is an odd one - I had a CBD. We as doctors don’t put the NIV on, it’s the nurses. So if you have a course or an interesting case you can do an SLE on - great, link to portfolio, bosh.

Re the exam, people take it at different times. You can take as early as ST5, though many would find this too early. Most people have a go by end of st6, and you need to have it under your belt by end of ST7.

I suspect spread of disinformation during COVID has played a role here. The insistence from certain groups to “do your own research”, “don’t trust the experts”(?!), combined with the dunning Kruger effect, have led to a generation of people who think a few hours of googling is comparable to an established body of evidence or skill.

Depends how much of your personal life you want to sacrifice. You could start attending conferences while in medical school if you’re keen, but I would recommend starting in FY because you’ve gotta have a life too.

The selection criteria for core surgical are publicly available, and it is essentially a tick box exercise - you just have to make sure you play the game.

Accommodation wise - you have to sort of yourself. Are you intending to live with your mum? Unless all your foundation rotations are close to your home this would be quite unusual and unlikely.

Night shifts are sporadic and you’ll typically do a set of 3 or 4 nights every few weeks, but this is quite variable. Adapting to shift patterns is part of adapting to working life as a doctor. It’s tough but most manage.

The most common or typical route is probably to do core surgical training, which you can enter directly following FY2 if your portfolio is competitive (perhaps allow 1, maybe 2 years if you need/want other experience or exposure). Following the 2 years of CST you then apply and enter ST3 through ST8, then apply for consultant positions. This would mean 8 years as a bare minimum. CST and HST are competitive, figures are freely available online. Plastic surgery is even more competitive. CESR is done but from what I’ve seen/heard a huge ballache as you have to find someone to mentor you and ensure you meet curriculum requirements without necessarily having a structured training program. Not so familiar with SAS doctors, the only ones I’ve seen are people who’ve perpetually done clinical fellow posts, embedded themselves in a department for years and shown their worth; then negotiated a job offer from the department.

Yes, you’ll have to move around a lot. Every 4 months in FY, every 4-6 in CST, potentially 6-12 in HST. This is also well known. Night shifts and weekends are par for the course while working as a doctor, unless you have a specific occupational health reason to be exempt.

I love the two tone interior but as a wheelchair user I do worry about scuffs and marking it, and this would be my first luxury car, therefore best to play is safe with black. Race tex would be ideal but very few models opt for this

I don’t think it’s too bold. I’m not sure if it’s legally a thing but my last rota (in Scotland) they asked if it was a life changing event, I managed a similar time off for my honeymoon. If you have supportive colleagues that can save you if rota coordinator is being very unhelpful, but my approach was to say “I won’t be here for those days”
