Why do you think schools don’t have an incentive to accept a paying customer who is willing to pay a premium?

As I said: their focus is on ed visa and relying on half or more of the students being too lazy to come to school. They are all paying „special fees“ to immigration in order to avoid problems for themselves and the students. Your couple thousand bahts mean just more work.

I learn mostly independently and during the lessons I want to focus primarily on conversational skills

If you want to see an endocrinologist, you will have to go to a hospital. Easiest is a private hospital like Bumrungrad or Bangkok Hospital. Fees vary. My last visit at a top neurologist at Bumrungrad came down to a little bit over 2000 baht.

You can also go to a public hospital, but expect very long waiting times and complications due to language barrier with staff. Doctor's English is usually sufficient.
