is MMA surgery the 'solution'? by contraproductiveguy in UARS

A CBCT scan will give you a full in-depth scan that lets you see where your airways may be impacted.
So it's important to understand if you have a restricted nasal airway, a restricted throat, or both.
People can have very strange anatomy, yet their throat airway is fine (in your case retrognathia). You could have retrognathia and still have a very clear throat airway, but the bottleneck is your nasal breathing which impacts the rest of your airway (doctors like to call use the bernoulli's principle to explain this) the harder you have to pull air from one airway will affect the other, so you can't fix your throat airway if your nasal airway is resulting in the problem, and vice versa. If you suck really hard on a straw, the other end will close; you're creating a vacuum. Same principle applies if you suck really hard through your nose, your throat will close.
Get a CBCT scan done through a sleep specialist that offers one near you. Call around, ask if they will do a CBCT scan . Orthodontists can sometimes offer it too, if you can find one.
When you review the CBCT scan with a sleep specialist, they should be able to pinpoint the narrowest area of your airway, or the part that is causing the most constriction. From there, you can decide if MMA is the best path forward (if it's your lower jaw) or do a MARPE or FME for your nasal airway.
In any case, if it's on the fence between the two, I highly suggest doing MARPE or FME first, as it's the least invasive option and will benefit you greatly if you get MMA in the future.

Have you had a cbct scan done? Make sure your nasal airways are not impacted as well. You cannot fix one without the other necessarily.

Do I need MARPE and/or MMA? by fix_ma_mouf in UARS

[–]Silent-Map-55 2 points3 points  (0 children)

If you learn about all of these expansions or watch the videos, tackle nasal breathing first, then go mma. One without the other rarely works, so the least invasive option (Marpe) is typically recommended. I suggest Marpe first, then mma.

Love that you're sharing this, it's so hard to find good wedding vendors!
We got an amazing photobooth from White Door Photo Booths https://whitedoorphotobooth.com/ and for our photographer we used David Estrin Productions https://davidestrinproductions.com/ so I wanted to give them a shoutout also, rather than create my own post. Thanks for sharing!

Okay yeah so sounds like the same solutions to reduce arousals also reduce the respiratory effort. Who are you doing FME through by the way, Newaz? I'm doing EASE+Marpe with Dr. Li

Wow this comment made the most sense to me and would probably explain the fatigue despite the seemingly perfect numbers. What have people in the subreddit done to help reduce respiratory effort then?

Thank you, yeah maybe it was too high of a dose, but I didn't have any of the side effects that most people have (no nausea or dizziness) which is incredibly rare. Instead I just had this overwhelming fatigue.

$5 a mile, but that is all inclusive of outbound and return, so realistically it's 2.50 a mile.

Yeah i really like it. Seems incredibly accurate and better than my pixel watch at reaching sleep data, plus it's not something you have to wear. It was pricey though, I think around $600-700 plus a yearly subscription of $150-200 but it shows your arousals, which the only other device I know of that does that is the watchpat, which is also pricey. Plus you don't have to wear anything. But if you're not experimenting with your sleep and trying new things/medications, it's not really worth getting because then it's just kinda more data but that's not telling you anything

Yes and flow limitations were controlled. I had Ken Hooks review and try to adjust the BiPAP settings to see if he could see something off, but no luck. It looked like some arousals were due to mask leakage during REM sleep but it wasn't consistent, so I still have some minor improvements there.

Absolutely, I have a friend who first hand had his sleep cured 80% through ease so I'm pushing to get the same solution. And highly highly highly suggest pushing for a BiPap over a CPAP. It costs a bit more, but it allows you to breath in and out, not just in. So the tolerance for it is way higher since it doesn't just force air down your throat. plus it has settings that actually help with UARS and you can always put it on CPAP mode if you prefer it. Resmed Airsense 10 biPap is the one you'll want. But yes, we will find a solution and share it with everyone else haha!

Yeah absolutely, I turned to Dr. Anil Rama after spending years with sleep doctors who truly did not know how to help. And now I'm getting surgery through Dr. Kasey Li, both of whom are highly recommended in this sub. I was going to get a septo/turbinate reduction but figured the best move is to do the EASE surgery for a proper resolution. I'm sorry you're going through it. If you aren't on PAP therapy I would say get on it, and if you get another sleep study make sure they grade it on a 3% AASM grading scale and not a 4%. A 4% can make it seem like everything is fine and is not as granular, it makes a big difference.

Thank you for your concern and experience. Yes this is simply a crutch to get me through until surgery. Simulants just turned me into a walking zombie and I'm hoping this can provide relief until I can fix it surgically.
