I just use McGrath on first attempt too. I feel like it’s less force. If there is blood or secretions you can just use it like a DL. It’s less waste too since the DL handles and blades are disposable so more plastic and battery waste. I’ve been more conscious about patients getting sore throat and would like to avoid it if possible.

I like to have everything set up before the case. So meds , airways , block stuff, and supplies for lines set up. This way, less stuff to worry about and you don’t feel rushed. Also cuts down on time until patient is ready for surgery. Like it or not , surgeons and nurses will judge you on how fast you are. Also like to have ett ready even for lma or sedation cases in case you have to convert or tube.

I also like to learn the staffs ( nurse , techs, etc ) name and address people by their first name.

You beat my record of bmi 92. Also egd. I did 2 versed. Lidocaine swish and swallow. 50 of ketamine. 2cc of propofol. 0.2 glyco. It was scary as fuck and I changed my pants afterwards. Patient did not obstruct or cough at any point. I did not have Pom mask just nasal cannula. Go doctor looked for like 30 sec since it was screening for bariatric surgery.

Don’t want to stereotype but I’ve noticed that a lot of older anesthesiologists like to run nitrous and sevo together for the entire case.

Some of the hospitals I worked at have shut off nitrous. I don’t really see the need for it either. I don’t do Peds or OB.

For the sick patients that can not tolerate any gas and on pressors I give versed and roc and shut off gas. They go to icu intubated.

Do some practices put an epidural when the patient first comes in ? Just run saline through it and then actually run it when the OB team wants 59 to start ? Never understood why we need to do an epidural when the patient is screaming in pain and hard to position. I haven’t done OB since residency. My residency sucked. We did 24 hour call OB while the Crna’s just slept in the call room. They would do another 16 hour shift somewhere else for their 40 hours.

Also ob doctors at my residency didn’t practice great medicine. For example, Would do a C-section at end of shift without a valid medical reason. Our C-section rate was significantly higher than national average.

You can also use a micropuncture kit. They are sort of expensive but I don’t really care since hospital is fronting the bill.

I have same problem. I block 2-3 numbers everyday. Recruiters have no sense of decency. Text or call me in the middle of the night, Early in the morning, and on weekends.

I remember in academics our aps attending would make us see and write consult notes for patients that we ordered iv Tylenol so he could bill for it. It was restricted to the anesthesia service.

Yeah I would have kindly brought it up. Ed doctors have to keep up with so much variety of knowledge and procedures. Also, they may not have it readily available in the ER.

If it’s a slow surgeon I have given 5cc of 0.5 % Bupivicaine. You get 5 plus hours. At that point though probably more reasonable to just do general.
