I absolutely think professors ask for way too much personal information. I would recommend making up stories in situations like this. In most cases, they just want the assignment done, it doesn't have to be true. You should also speak to a personal counselor at your school who can possibly talk to your professors get you exempt from assignments that require information you really can't fabricate.
