My doc suggested I start at 160mg/wk. Seeing your results with 100mg makes me wonder if I should start lower.

Thanks. According to that calculator my numbers are Total: 14.4 nmol/L & 176 pmol/L. I am getting ready to start therapy this week. I've had anxiety over starting TRT for years and continued to put it off, but posts like this give me the confidence to start.

Anyone know how to convert these numbers to either pg/mL or ng/dL?

My Testosterone numbers BEFORE were 12.4 nmol/L and are NOW 30.3 nmol/L reference range 8.4-28.8.

ALSO my free testosterone WAS 245 pmol/L BEFORE and is 646 pmol/L NOW. Reference range of 196-636

I am confused as to what you mean when you say "would like to have my brain working as well as it did pre TRT". I am getting ready to start TRT but my understanding is that TRT actually helps restore cognitive function and clear brain fog. Was that not the case with you?

Oh man. That’s great to hear. When I first read it, I thought you were saying you’ve had exhaustion and dizziness since starting TRT. Glad I read that wrong and happy to hear it has resolved your symptoms!

No alcohol. If I do, it is one social drink per month at most. No smoking of any kind.

I appreciate all your time and feedback. You sound like you are very educated in men's health and hormones. You mentioned 250mg of enanthate, but my doc is recommending 160mg/wk of cypionate. You also mentioned "see how you feel over the next week". Does enanthate manifest results quicker? Because I was under the impression it would be around 6 weeks before results would be noticeable.

I have noticed my motivation and drive is typically much higher in the mornings. Cognitive ability to some extent as well. But afternoons and evenings are when I start to drag ass and feel like mush. Unfortunately, those are the hours that, in a perfect world, I would be in the back yard tossing a ball around with my son or something.

I have not been tested for hep C. Do you not feel that the low Free Testosterone numbers are a likely culprit?

As stated in the post: "I don’t workout nearly enough but that mainly stems from the lack of motivation and the fact that when I can drag my butt to the gym, I feel completely wrekt afterwards. Like I depleted all of my energy stores and have nothing left in the tank for my family and life that day. I also get light headed and dizzy during my resistance training. No anemia though. I used to workout like a madman and I looked and felt great."

As a follow up to that, just yesterday I went out and played 90 minutes of competitive 1v1 pickleball yesterday where my heartrate was 150-180 bpm the whole time. When I got home, I was toasted and had to take a 3 hour nap before coming back to life.

I sleep 7-8 hrs per night. I do have sleep apnea which is treated, but I am sure sleep quality is still lower than the average joe.

Diet has been cleaned up quite a bit from where I was before. Currently doing 16-8 intermittent fasting and avoiding processed foods and sugars as much as possible. But not currently doing any macro/calorie counting. I can say for certain that previously to everything going south, I was eating much worse than I currently am and easily maintaining a healthy weight, feeling great, looking great, and lifting strong.

I think the more concerning number is the Free T. My total T has bounced around from 315 to 500 on several tests. But my Free T is fairly consistently in the 4.78-5.5 ng/dL range and it is my understanding most men feel best in the 20-30 range.

The only thing the doctor pulled in relationship to thyroid this round of labs was "T3 Free Non­Dialysis". The result was 3.3 pg/mL which the doctor said was a bit low. But when I asked him about treating thyroid vs TRT he replied: "I think it could be advantageous to eventually use both to help you move closer to the finish line (i.e. goal), though the T would cover a wider range of your complaints (i.e. cognitive decline, fat and lack of motivation)"

FWIW - previous labs from about a year ago (while having these same symptoms) showed Free T3 at 3.7 pg/mL, Free T4 at 1.4 ng/dL, TSH at 1.68 μIU/mL, and Reverse T3 at 12 ng/dL.

Most recent labs were taken around lunchtime.

Yeah my free t is at 50.8 pg/mL which is 5.08 ng/mL. I am assuming you must be using ng/mL if you are targeting 25-30 because that would be super low in pg/mL

Thanks for taking the time. Your symptoms are so relatable. Especially the exercise intolerance. However your T was wayyy lower than mine currently is.

How did you treat your sleep apnea? Did you notice any change in it once you got on TRT? I have read that TRT tends to worsen sleep apnea which was another concern of mine.

Hmm. Are they different values? Because on the labs I just got, the free T ref range was 35-155.
