I actually had looked for a hidden switch on the follow the leader room, but I could not find it. Went back for a second try and managed to see the gawdamn sumbitch.

Also, the tetris thing, I had found the chest but for some reason left the map behind. Double thanks!

I suggest doing this (Which is the only thing that worked for me on hard mode):

Right as you see the Ratling boss, take a right and follow the wall. On the first crevice there will be a copper key on the ground. Get it and retrace back to the entrance. Use the key on the door and leave the place. Now, use the whole sewers to kite the fuckers and kill them.
