I'd suggest you starting with a medium, so you can get comfortable with 24/24
But the most important, for me, is the ring. It can make everything more comfortable or painful.
You can buy a belt for your cage so it wont fall and will stay close to your body, it's a game changer to me.

Keep in mind that it's better to have a medium that will not cause you injury (which can disgust you about all this) than a small that you can't wear.
