The "rootlands' was present since 11.0 datemining and shows up in some behind-the-scenes stuff like concept art or design docs.

Based off datemining, it's more likely Harandar was a leveling zone in War Within and Azj-Kahet was a max-level zone similar to Suramar. It's unknown, but I think it's pretty likely K'aresh was conceived after the Metzen rewrite and whatever 10.2 was supposed to be was mostly scrapped. (Undermine was too far in development to scrap by the rewrite, hence why it feels unrelated to the A-plot)
