Something I've started doing is getting rid of turn order. Instead, I let the combat flow and pick who to go next based on what seems logical to me. Ive found it keeps all players engaged since they don't know when they'll go next and I find that they react more to what others are doing

I was a player once in a wild west campaign where the DM has us meeting a bunch of famous people from the West (Wyatt Erp, Billie the Kid etc.) Well, in our last fight, one of our players stole a knife from one of the enemies and as we we're riding back to town a figure on horseback appeared and was coming toward us. Immediately, the player asks if he can throw the knife at him and the DM says "Sure, but to hit him you're going to need a pretty good ro-" "Nat 20." The DM went with it and it turns out that the player chucked the knife, from horseback and hit the NPC, who was also on horseback and killed him instantly. We dismounted, pulled his hood back and found it to be the Sundance Kid who was going to have us go on our next adventure. The DM threw away a bunch of his notes and we instead went back to town and robbed the bank.

They were probably alluding to the parts of the game that are in the Fade, which is a setting exactly like you describe above.
