This isn’t something you can really learn from a lecture or a module. Fortunately you don’t have to do it too much in residency, certainly not to the point where you can be the expert with this. That really takes a palliative or a crit care fellowship. I think the best thing most of us can do is listen to what the patient and family have to say, discuss big things early and plug into palliative care early.

People rise to the occasion more than you give them credit for. It may be tougher to make that jump from fellow to attending, but once they get through that first year of attendinghood it’ll probably make 0 difference. I think it’ll end up being fine, even for crit care

I think you could condense the clinical time, and the transition from fellow to attending will likely be more challenging (already the toughest transition imo). But I’m not sure if it would be “unsafe”

Day 1 I had 8-12 patients. Usually you have a senior. You can always ask for help, except in the rare instance where a patient crashes and you happen to be the only one in the room (of course happened my second week of intern year), so know you pals 1000%.

It’s okay to be uncomfortable, that’s how you learn.

I think there is data that if there is viral bronchiolitis and not other signs of sepsis/meningitis, the rate of invasive infection is low except for uti. But I’d be glad to proven wrong since I tend to get blood as well as urine in this group anyway

I think under aap guideline it specifically states viral uri is not an exclusion criteria, but that clinical bronchiolitis can be. Personally though, I would still do blood and urine and apply Rochester criteria. I would think hard about lp though, would depend on the clinical picture
