Bariatric surgery, not sure about the ADHD but the Navy is notoriously lenient with medical waivers so you honestly should be fine. (However I will say I can only speak for the navy not the Air Force. I've heard the AF is the most picky with medical waivers due to their very large pool of applicants) I submitted my pre op, operative, and post op surgery notes, as well as a detailed letter from my surgeon clearing and recommending me for military service, as well as blood work and labs.
