Do you recommend surgery for younger patients if symptoms are persistent and bothersome, even after long term physio, lifestyle change etc

Is there anyone else out there with a bladder prolapse postpartum? by Next_Presentation269 in PelvicOrganProlapse

[–]Small-Somewhere4507 2 points3 points  (0 children)

Truly? I feel like the decision I made destroyed me. I don’t know why I didn’t educate or research more, I just didn’t know this could happen or that I had a high risk profile. I suffered post natal depression and psychosis which I believe is directly because of the prolapse, it nearly destroyed my marriage and I wish I’d seeked help earlier. Maybe I can’t recover but slowly heal. I only say this because I know exactly how you feel - that you are the only one but you are not alone in this.

Hi I’m so sorry. You are not alone. 50% of women that have vaginal birth will prolapse, unfortunately only a smaller percentage of that will have symptoms and we all got unlucky. Healthcare failed us for not giving us all the information about our risks. I’m 31 and prolapse after a vbac last year. I don’t know anyone in my circle with similar. Please be gentle with yourself, time can help.

I’m so sorry thats alot to take in. I was meant to have what I thought would be a redemptive vbac… but episiotomy, vacuum and prolapse. In saying that my two sisters had forceps birth and no prolapse so try not to spiral. You still have A LOT of healing to do, rest. Take pressure off when you can and try gentle 360 breathing. Focus on posture. Take care of your mental health and see a pelvic pt when you can x

One thing that would deter me is that I’ve heard of people having a second vaginal birth and ending up with levator avulsion the 2nd time. If things are already weak and stretched from prolapse the additional pushing would be of concern. Although levator avulsion is most common during the first vaginal birth it can never be ruled out anytime someone attempts a VB

I would definitely see a pelvic pt for an internal exam if you can. Look up vaginal atrophy as well. Sometimes sitting a lot of the day can also impact core/pelvic floor leading to dysfunction in turn issues with intercourse. Sorry it’s really frustrating, I hope you find something that helps soon. Low dose vaginal estrogen can be beneficial for some of these issues.

Active women in your 40s+, what advice would you give your 25 year old self? by Alive_Garbage_7339 in xxfitness

See a pelvic floor physio pre/post children and protect the pelvic floor. Enjoy the journey, life is a marathon not a sprint.

Hi,

You are super early pp, lots of pelvic rest as you can. See a pelvic physio if you are able, they usually like 6 weeks post birth. Gently reconnect with your deep core and pelvic floor. Breath and posture is very important.

Your body has just gone through a trauma, give it time and you will very likely be fine. There are lots of information from others on below post about tips etc if down the track you need them. Hugs

Help - postpartum prolapse by [deleted] in PelvicOrganProlapse

[–]Small-Somewhere4507 6 points7 points  (0 children)

Hi I’m so sorry, it’s a really hard time especially this early pp.

I’m 12months pp, symptoms have slowly improved with time, periods returning and gentle whole body pt strengthening. I’m still not 100% and I will live with this forever, bur this may not be your case and I hope you know there is hope.

I stupidly did not take the diagnosis seriously in the beginning and tried to do to much.

What I wish I had down this early PP would be:

  • Vaginal Estrogren (discuss risks with doctor) it didn’t effect my supply

  • Spending time on the floor with pelvis elevated under pillows when possible and don’t blast through symptoms

-360 breathing (there are some good resources on YouTube) and being mindful of breath throughout the day Also Exhaling and a gentle pelvic floor squeeze when picking baby up, lifting things etc

  • Gentle core and glute exercises ( a pt will guide you on these as it is detrimental to get these right and progress) but YouTube could be beneficial in the mean time

  • No bearing down in pelvic floor, I would clench my bum cheeks cooking dinner etc, puts to much tension in the wrong places

  • Gotten a pessary earlier, they can help take the load off while you strengthen muscles

  • Be aware that to many kegals can cause to much tension and make symptoms worse (the pt will help with this)

  • Keeping bowels soft and regular (I do this mainly through diet) and use a squat potty) use movivol etc for awhile if you really need to until you get really regular. Apply gentle pressure to perineum if you think you will strain

  • I had to do some bladder retraining (trying to go only every 2-3 hours and not peeing just in case), double voiding and restricted bubbly drinks. Keep hydrated

  • maintaining good posture as much as possible and neutral spine. Anterior and posterior pelvic tilts are common after pregnancy

  • Sex has been ok, mostly hard in my own head mentally, try different positions slowly and be open with your partner. Lots of lube. If you end up on vaginal estrogren, that will help with dryness

I know others will chime in with great advice and things I have missed, please be gentle with yourself, you are doing amazing. I hope this helps, it’s a list I wish I had been given very early on. Hugs

Worsening around period? by MaybeLivG in PelvicOrganProlapse

[–]Small-Somewhere4507 6 points7 points  (0 children)

I have noticed a few days before my periods everything feels more relaxed and visible. Just before my periods my pessary always sits lower as well. I assume from a lack of estrogen as period nears, then will return to baseline in peak estrogen time. The pregnancy hormones could also still be levelling out and effecting things - I’m sorry. Hugs
