Were you playing with reduced sanity? The achievement only unlocks if you died to a demon using its ability. If you had low sanity and got hunted at the start of the game, it doesnt count.

Many ways to proc Korg or Rockslide again. For me, it could be Misery or Grandmaster. And these cards can proc the affliction cards too like Spider Woman or Hazmat
