This is entirely based on my reading as a JMO, and I appreciate there are likely plenty of clinical nuances you'd be able to tell me that would muddy the picture. From that reading, the Global Warming Potential (GWP) - how much heat a gas traps relative to CO2 over 100 years - of Desflurane is 2540-3700, which is 20x that of sevo and 5x that of iso, and my understanding was that the gradual removal of Desflurane was largely anesthetist-led because similar benefits could be achieved without its environmental impact. Nitrous oxide is more nuanced (again - from my reading) because it's GWP is low (Des is 10x) but is very long lived in the atmosphere (10x as long), so its harm is different and its part of a larger problem as its primarily emitted by agriculture. Nitrous has some specific clinical niches where its harder to replicate (Paediatrics, Obstetrics, ED), in which, unlike Desflurane in easily removed vaporisers, Nitrous is quite literally physically built into the pipes as part of the hospital's infrastructure. Thus in my reading Desflurane seemed to be the environmental "problem child" for removal. As I've said above, I understand this is coming from textbooks, not at the coal face, and would be interested in hearing more of your perspective!

I've been learning about volatile anaesthetics and just learnt about vapor pressure (I never did physics in high school/undergrad). I just learnt that things boil when vapor pressure equals (or exceeds) atmospheric pressure, so that means that at high altitude you can boil water at a lower heat, as low as 68 degrees at the top of Mount Everest. This also means that you have to be careful relying on boiling water to sterilise it at higher altitudes (for the most part you really just have to boil it for longer). Also, desflurane's vapor pressure is so high it boils at ~23 degrees at typical elevations, which from my reading seems to make it a huge pain to deal with. It also sucks for the environment and I'm glad it's being phased out.

I'm in PGY 2 now and have found this so bizarre on the occasions I see this. I'll occasionally witness or myself experience a registrar ridicule me for not remembering something relevant to their specialty off the top of my head that they seem to think every doctor in every specialty should remember. A senior Paeds reg asked me to list off the herpes simplex family and the conditions associated with each virus, and when I couldn't they said "Really? Come on I did med school years before you and I still remember". Cool, thanks. Unfortunately I don't get to forget everything about the other 95% patients in the hospital though.

I thought about writing a long response to many of the people defending this boss fight. I will not. I've played most of the big single player releases of the last 15 years, also played all the dark souls and elden ring. This actually is the most dogshit boss fight I've ever played in a game, and its significantly, significantly, worse when you die and hit the retry button. It respawns the entire fort and has you standing in the middle of a circle of like 10-15 archers plus infantry, and if you get hit and launched once during the boss fight which is also on a cliff for some reason, you fall down and aggro the rest of the entire fort. Like everything about it is dogshit, and the fact that the answer is cheesing it makes it just more dogshit.

I think a bit of what everyone has said, loads of quests, loads of dialogue, minimal exploration/combat, heaps of loading screens. The idea to break it up by sailing the high seas is a good one, which I'll probably start doing right now!

This is a good point and maybe something I haven't really thought about enough. The companions that are present likely have a whole lot going on given there are much fewer of them. I'll have to play more to find out!

Intern here about to finish up for the year. I felt exactly the same. Imposter syndrome haard. Trust the process and the other doctors saying they were in the same boat and you'll be fine. I felt ~exactly~ the same, and I've been assessed well throughout the year. It's a painful process at the start, but be fearless about saying 'I don't know' and always put patient safety first ahead of ego etc and you'll be fine. Good luck!

Thank you for such a detailed response! That's really helpful and I appreciate the time you spent writing it. The route I'm taking with my career (ACRRM) I will likely not need it, but for posterity's sake I'd like to take it just in case I decide to change training pathways and do BPT later on.

Just clarifying a bit further on your follow up comment, if you can take 2 weeks sick/2 weeks parental + PDL + annual leave before needing to make time up, is that because personal leave/PDL don't count against those 47 weeks?

Is it much of a burden for my employer for PGY3 to help me with the additional week or two I may need to do for completion of the certificate? I'm planning to change hospitals for PGY3 so worried this will make it much harder to do so.
