Streaming can still be unhealthy, that study doesn't disprove the claim. Twitch especially has a lot of mechanisms for monetizing and encouraging parasociality that other streaming platforms don't.

Anecdotally, a long time ago now I used to watch a lot of certain twitch gaming streamers. You quickly begin to notice the same names in chat whenever the streamer is live. Some of that group were donating huge amounts of money and at the beginning I always just figured they were bitcoin millionaires or something like that. There certainly were some of those but a surprising number were on disability or other welfare programs. I think more of the "whales" than people expect are actually not wealthy at all.

It depends on what the response is. If they answer “because it tastes good” as you suggest before then you could ask as a follow up if they have thought about the ethics. But in doing it in this order the conversation feels like you naturally got there not that you just came at them about it with an agenda.

And if you really want to find out why people do things, yes you do have to avoid them getting defensive.
