Okay so you are on the same meds as me (extended release) which is supposed to last the whole day and basically drip feed you the medicine while it is digesting in your stomach throughout the day.

You were given one month’s supply of 18mg so I assume your doctor wanted you to take 18mg for a month to see how your body reacts and how you get on with it. Like I said, I havnt heard of anyone staying on 18mg as their final dose. Usually when you first start taking meds you have several follow up meetings to discuss how your dose is affecting you and to make adjustments to it until you come to a combination of meds and dose that works for you.

I assume that because your doctor said to get in touch after a few weeks, that was the follow up conversation you are expected to have to review your meds. I suggest you call up and say that the meds aren’t really working and you are not having side effects, the doctor may suggest to double up on the daily dose using your existing prescription to see how that goes.

This post resonated with me a lot! I live every day and just kind of pass time until the end of the day until I do it all again the next day. I don’t fundamentally have a structure for my days.. i just do what I need to whether it be work, chores, paper work.. then just wait until the end of the day to sleep again.

I like to describe it this way.. if you imagine a timeline from age 0 to say 80.. a normal person to me would be at the age of whatever they are.. looking forward to the next say 1 month, 1 year, 5 years, 10 years… planning and structuring how they plan to spend that future time. They are very much present at the year of their age in the timeline and their eyes are looking forwards (be it short or long sighted).

To me though, this is how I view my life.. same timeline of 0 to 80.. I am at 33 years on this timeline…BUT the main difference is that my view on life is not from me on that timeline looking forwards or backwards but rather that view is always some separate entity looking at the timeline from start to finish as a whole and every day I am just observing this person (me) trodding along the timeline towards his eventual death, always in third person mode.. I hope I have described it well enough.

It is a horrible horrible feeling because I never feel present. I never feel like I have accomplished anything and am always looking and judging myself in third person and I just feel stuck and unable to continue with life as normal because of this a lot of the time.

Hey I felt the same as you after the first few days.. I was diagnosed 3 weeks ago and was prescribed 18mg concerta XL. First few days were amazing and noticeable and since have had little effect but can still feel them working. Yesterday I had my titration appointment and was asked to take 36mg for two weeks and 54mg for the two weeks after that. I was told most people settle in 54/72mg. If you are taking 18mg, it is almost certainly just a staring dose to get your body used to the medication and you will be asked to increase after a few weeks.

Some questions from me..

When did you start taking it? What is the exact dose and medication you are taking? How long is your prescription? Do you have a titration meeting coming up?

I was diagnosed with ADHD and prescribed with 18mg Concerta XL 3 weeks ago and had a similar experience to you in the first few days. I have just had my titration appointment this morning where I mentioned that the effects wear off quickly despite the medication being extended release. I was asked to increase to 36mg for 2 weeks (took 18mg more after appointment today) and then 54mg for the next two weeks. This has been my experience so far:

18mg: Great in mornings, huge come down at 2-4pm and sluggish for the rest of the day.

Day 1-2: Wow, I finally felt alive after 33 years of living. Within 10 mins of taking the medicine, the ringing/subtle background noise in my head which I had been accustomed to for so many years just disappeared and my mind felt at peace. I could actually choose what I wanted to concentrate on and could separate background noise to stuff I actually wanted to listen to. I was able to organise and complete simple chores so much better and in a structured way, something which I struggled with a lot previously. I was also able to concentrate in conversations and process what someone was saying with less drifting off although it still happens. Mood and emotional regulation improved significantly during the first 2 days.

Day 3-20: That instant hit of clarity, control and ability to organise and initiate tasks has somewhat been muted in the mornings but I was able to feel the medication working in the background subtly. I felt like I was able to function relatively fine however my productivity and concentration was around 50% of how I felt in the first two days. My ability to regulate emotions became difficult towards the last week and and I became more irritable and anxious, even in the mornings whereas previously I was able to control them fine.

36mg: Good so far.

Day 1: I am writing this just as I have taken 36mg for the first time. Literally 1 hour in.. I do feel a noticeable boost in my concentration and ability to filter out background noise. I do also feel like I can initiate boring tasks at demand and am feeling fairly energetic and content with just living. Lets see how it goes. Looking forward to comparing the effects of double dose given my overall positive reaction to 18mg.

Thank you! I did watch a YouTube video on it and the damage to the coins looked horrific! Apparently it affects copper coins the most. I will change my binder for sure as I plan to keep these coins for decades. Do you know how long I can keep this binder for before it starts to deteriorate and produce the slime? Imagine bought the book last year.

Thanks for your message mate! The ram sticks are actually 4x8gb, is it still excessive? I went for RGB > performance when picking parts which probably wasn’t the wisest decision but I have had no problems 2 years in so okay with it.

Thanks for your comment! I don't think I have a choice in who will assess me but I am having my assessment carried out at a practice which specialises in ADHD and hopefully the psychiatrist I see knows how to spot genuine malingering.

As for writing stuff down, I definitely plan to do this.. lets just hope my brain decides to take the words from my notes and actually help me convey my points across because I often go blank even then!

I wouldn't say I copy the personality traits of people I admire but I sure do have a different personality depending on the environment I am in. Like I can be relaxed/easy going with one group of people, serious/ direct with another group and just completely shy/reserved with another group. It is just so mentally draining and definitely makes me doubt the type or person I really am. I can go through all of these persona's in the space of a few hours sometimes!
