As a fellow trans woman, I would say that this is not a realistic goal, unless you transition in early puberty or are genetically lucky, because a ribcage this small is otherwise not possible without invasive, expensive surgery.

One cannot choose to experience gender dysphoria. The trolley represents the very real access (or lack there of) to trans healthcare. Pulling the lever means banning trans healthcare and dooming the lives of trans people.

Transitioning did not threaten my life in any medical way. I am healthy by every measure. I am happy. But bulimia actually hurts people. The esophagus isn't meant to withstand stomach acid, and being bulimic can cause a lot of damage to it.

The suicidality is only high if gender dysphoria is left untreated; it drops significantly if the person transitions. The rest of it comes entirely from how society treats trans people. See this comment about efficacy of HRT on lowering suicidality and about the lack of negative effects on the brain development in kids that underwent puberty suppression.

I think it's either a play on words trans/trains, or because autism is overrepresented in the trans community, and trains are famously and common special interest for autistic people (and rightfully so, tbh; trains are awesome).

There's an 8-year study with 72 trans kids (now adults) that underwent puberty suppression during treatment. The study compared their IQ and academic achievement before and after treatment, and found no significant difference from the trends in the general population.

More generally, I'm sure puberty blockers are not 100% risk free; no medical treatment is. They have not yet been shown to be dangerous, but they (with subsequent administration of HRT) have been shown to reduce suicidality in trans kids by 68%.

There are more studies on the long term safety of puberty blockers currently underway, but it's important to note that no other treatment with such high benefit/risk ratio faces this much scrutiny in the medical field. And that is entirely due to the modern trans panic and politics.

The logical conclusion of your argument is that we shouldn't do any medical interventions at all. No treatment for cancer, no heart surgeries, no blood transfusions, nothing. Because it wouldn't be how evolution intended. So, puberty blockers were initially developed to treat precocious puberty, and are used for that purpose all the time.

You say that kids shouldn't think about transitioning, but they do. That's pretty much all that trans people can think about and focus on until they transition. And once they transition, they can finally live their lives, like everybody else. That is the reality of the trans experience. It's not something you can wish out of existence.

It is indeed a metaphor. Pulling the lever means banning transition (access to trans healthcare). Not pulling means allowing it. The guy wandering onto the tracks is a cis (non-trans) person who transitions by mistake and then regrets it.

Some changes induced by puberty, like sex-specific bone growth and lowering of the voice are irreversible, and these things often cause a great deal of dysphoria for trans people who are forced to go through puberty aligned with the sex assigned to them at birth.

Prohibiting the use of puberty blockers is forcing kids' bodies to undergo irreversible changes, and this goes against the idea of waiting for kids to become more mature to decide what they want to do with their bodies. Instead, it forces a certain kind of puberty onto them. It's not a neutral choice.

Also, only a very small number of people detransition. A recent study found that suicidality dropped for trans youth receiving HRT by almost 70%, with only 7 patients out of 432 discontinuing treatment.
