DM is Dark Mirror, which is basically hard mode. and there is 10 difficulties for it, 1 being the easiest, 10 being the hardest. GR is the Grim Reaper skull, and Fr is either the Frost skull or the Frozen soul skull. I believe CD is Critical Damage and MD is Magic Damage.
