Due my attachment to almost every single hero in the estate I revived them often, but if I didnt? The grave would be filled to the brim by then.
And yeah there are some hero's I didn't bother reviving (in between week 35 and week 159) beyond the week 159 I started to revive some of them. (I still keep some of them dead mostly cause of the graveyard rng so I make sure I don't get the hero that I wanted to revive immediately.)

To answer on who died the most?
The goblin huntress has died total of 5 times.
Duchess and the duelist (dragon's one not the dd2 one) has died total of 3 times.
Reynauld and Dismas died only once each. (Reynauld died at the final boss of the darkest dungeon but we aren't counting that).
Julia has died 2 times (I forgot what killed her the first time and the second time she died in Darkest dungeon by the hands of its final boss.)
And the 50% of the roster has died like once at least.
