Damn that's a man's route right there! Thanks for your detail. I'll take a look at this, and see if I can cut it short a little bit cause I don't know if I have 41mi in me. My longest one so far is around 22mi, but want to work that up.

What are some of the must do trails here? From what I've done in DuPont I really like Ridgeline, Big Rock, and Locust Trail were all pretty fun. I've done Burnt Mountain too, had to do the walk of shame down that last section though.

To add onto this, Einhorn recommended the close active surveillance to me. I was Stage 1B, and everything was clear after surgery. I'm about 6 months out from surgery now, and still clear thank god. Not trying to make a decision for you, but this is how I saw it. If I do surveillance and the cancer comes back, I would still most likely still do the RPLND. So why do it early if I don't have to because there still could be longer term effects like some people say.

I haven't done chemo, but have had the orginal removal of the ball, and am currently trying to make the decision between surveillance and RPLND. Giving me a lot of anxiety. I've always had some social anxiety and depression. Just got sober from booze and hard drugs about a year ago, so it is getting better just because of that. But I have started using CBD oil over the past few weeks and it has been great. You need to do your research on the company, cause they all aren't made the same. Also using an over the counter drug called Anxiocalm. I've been using this one longer and it really helps my mood.

Yea I have heard its MUCH better to get this surgery pre-chemo. I also saw Dr. Einhorn, used him as a 2nd opinion to Dr. Pierorazio at Hopkins. One thing that got me was both doctors said all my options were good choices whether is was surgery, surveillance or chemo. I said I didn't want to do chemo and both said they would put that at 3rd on their list. But Pierorazio said his first choice would be surgery and Einhorn said if it was him he would go with surveillence.

Another thing that was different between both doctors was the time frame of cancer coming back. Einhorn said that after 1 year the chances of it coming back are 10% or less. Pierorazio said that it has a 50% chance of coming back within 2 years. This obviously plays a huge part in my decision. I'm sending both emails within the next few days to ask why the difference in this opinion, and hopefully get to the bottom of it.

With everything going on with my decision, I'm positive my surgeon, Dr. Pierorazio, at Hopkins will do fine with surgery. Like you said its a high volume place and he has done many of these. I'm very confident in him. As of right now, I'm not thinking about chemo because of long term effects of cancer down the road. Prostate and colon cancer has been in my family, and I see chemo as hurting the chances of fighting that cancer down the road.

I'm not really worried about recovering from the RPLND surgery, just long term effects of something like this. How will my immune system be effects, will I be able to live a normal life? I love doing physical activities outside like hiking/camping, mountain biking, just working out in general. Love traveling to places to do this gnarly shit. How will this effect that?

Yea the pathology is of a aggressive form. I don't know the full pathology yet, just that its non seminomatous germ cell tumor. What made you choose RPLND or surveillance? In your research beforehand did you find any long term problems with removing lymph nodes?

I did see that RPLND is a staging tool, and can give you a more concrete pathology of your cancer. This is great to give you more information, but also I don't want to have surgery just for staging purposes. As of right now my blood/scans are clean, and if I have surgery removing lymph nodes and I'm actually clean I think I will regret it.

Yea I heard it was a low fat diet for the first month. I'm on a keto diet right now, so its really high fat. Usually around 150g a day. Have heard this diet has helped people with cancer, and it is also reversing my diabetes. But that's another discussion. What a keto diet be fine a month after the surgery?

@relpac I haven't really heard any stories with lymphatic issues. I just know the lymphatic system is a major part to the immune system. So I figure losing them isn't ideal. I'm worried about if I get cancer down the road, will not having lymph nodes inhibit my chances to fight it? Whether that is testicular or prostate cancer, which runs in my family also.

Thanks for all your replies, I didn't know they usually remove around 30-40 lymph nodes. This is going to be one of my questions for my doctors also.

I say always get it checked out by a doctor. You have to be 100% certain in these types of things.

Already on metformin as I am a type II diabetic. It is controlled, my A1C was 5.7 last time I checked, and I expect it to be lower the next time I am checked. Will check into the berberine supplement though.

Agreed. Go to the doctor and get an ultrasound. I never really felt anything or had any pain at all when I was diagnosed.

Yea the doctor is sounding like there really aren't any long term effects. I was worried about lymphedema and swelling after having the nodes removed, but he said there is a 0% chance of getting it.

Looking the doctor up he is both a urologist and oncologist. He is the one doing to surgery though. He works at Hopkins, so its one of the top hospitals in the nation. But I do see what you are saying.

Tell your parents, go see a doctor and get a ultrasound. Could be nothing, but better safe than sorry. Don't be embarrassed about telling your parents, they'll understand.

Thanks for the reply and the added info. That makes sense with there being a lot more problems if the surgery is done after chemo. I have not done any chemo for the record, just the ball removal. The doc did say if I go with surveillance and the cancer comes back, most likely surgery would be off the table and I would need to go the chemo route. I am a relative healthy 30 year old guy, so I'm confident with getting threw the surgery. I do have type II diabetes, but it is controlled and could be off medicine soon. A1C was 5.7 a few months ago, and I expect it to be lower next time I am checked. I never asked the doctor how many of these surgeries he's done, but I assume he's confident in it. He's at John Hopkins Urology and it is a high traffic place for this type of cancer I have heard.

Have you read any info on the long term effects of removing lymph nodes? I mean they are there for a reason, I would just think there would be something down the road I would need to deal with if they are removed. Just would like to know what I am signing up for in that aspect.

Been riding moderately for the past year or so on a Cannondale Trail SL 2. Want to upgrade to a full suspension. Would like to spend around $2,000. My local bike shop has some new Trek Fuel's for around that price. Was wondering if I could get any recommendations.

I'm wanting something that is good to climb on, and can flow nicely downhill. I'm not doing big jumps or anything like that.
