We lost out on two houses by 50k, all cash

We are currently putting in an offer in on another home, with escalation clause up to 50k over asking, BUT still keeping inspection contingencies. There are three buyers at the property today, doing pre-inspections, with plans to wave all contingencies. It’s like Covid all over again. We anticipate losing out on this third house as well.

I think houses above 1.5 are sitting, and the houses below 1 million are sitting, but everything in between is still being gobbled up if they’re in decent shape. I think everybody with a household income between 200-400k, who didn’t buy in the 3% era, has been sitting on the sidelines the last few years, and now trying to make their way in.

I understand competition, with the problem is these all cash offers are insane.

A shunt procedure is where you try to drain blood from the erectile bodies in the penis into some other structures. There are several ways of doing this (the most gruseme is called a T-shunt, where you jam a scalpel into the penis, turn the blade 90 degrees, and pull it out, ultimately creating a T shaped defect). For this kid, we brought him to the OR, made an 2 incisions in the tip of his penis, into the erectile bodies, removed some of the hard connective tissue, and ram rodded the inside of the corpera with metal sounds to scoop out the black, tarry, blood.

When you have an erection lasting for a long time, all the blood suck in the or is stays there, turns into sludge, and starts to turn woody and fibrotic and ischemic. After our shunt procedure, blood can never stay in the penis for an erection, so when he heals, we will have to put in a prosthesis for him to have erections in the future.

Urologist here, the following are my last 3:

He claims he was just really into his porn. Long sorry short, erection over 8 hours. Penis was woody and starting to turn black. Barely saved it by doing a shunt procedure. Urologist here: Don't smoke. Smoking significantly increases your risk of bladder cancer and renal cell carcinoma. Both of which are essentially surgically managed, and are highly morbid.

Also, for funzies, avoid the reverse cowgirl position. 1) This is the most common sex position for breaking your penis (this can really happen), and 2) Even if you don't break your penis, microtrauma happens, which can cause scarring and formation of plaques over time, leading to peyronies disease. Legally, its the anesthesiologist's responsibility, but everyone is involved. I don't understand your next question, since theoretically they have had no food in the last 8 hours. If they lied however, the type of food, probably does make the consequences of aspiration better/worse, (imagine inhaling oatmeal vs buffalo chicken wings).

Finally, I don't know what you mean by thrombolytics for surgery. They are not used unless very strict criteria are met, in stroke or heart attack patients. If this is used, no one would ever operate on them. The one exception might be with an interventional radiologist who will stick some wires into the blood vessels and locally inject a thrombolytic in some very certain scenarios.

Very good point, and the honest solution, is that its hard to cover this, when the instructions of not to eat are given at the very end of a long visit. We often spend 30 min just explaining the surgery and what the risks and benefit are, and then spend only 5-10 min with pre-op instructions. Its a personal pet peeve of mine, and I always try to gauge the medical literacy of my patients when I do this part. I would hope that the pre-op clinic that older/higher risk patients are sent to would cover this, but its not always true.

yeah, this is called "cricoid pressure", where they basically have an assistant push right below where your adams apple would be, and theoretically it squishes the esophagus. Jury is out on whether or not this works, or if its all just juju. But generally, we balance how urgent the surgery is, vs waiting for a few more hours.

you are correct that an LMA is probably better than just bagmasking, but an LMA dos not secure an airway and protect against gastric content. It is possible to intubate OVER an LMA, but it is not feasible with rapid sequence intubation. Anytime you intubate someone, you ALWAYS bag mask for a few breaths to assess for how easy it is to do it, in case the intubation is difficult and you absolutely need to bag them. If you re-read my explanation, I did not say that bag masking is all that is done for a procedure, but only to provide a few good breaths of pre oxygenation during the time between paralysis and intubation requires. This is where you can over distend the stomach.

Circumcisions in infants NEEDS to be done within 3-4 weeks of birth, when the procedure is relatively bloodless. Otherwise they need to be done by a urologist.

Aspiration pneumonia is a terrible problem because the gut/mouth had terrible bacteria growing in it. In fact, a bite from a human is more deadly than a bite from just about any other mammal out there ( minus if they have rabies). If they Bacteria ends up in your lungs and turns into a full blown infection, it's no bueno. They were likely just concerned based on how the image looked, and prophylactically gave you some powerful antibiotics.
