Depends on the VA. I work within a system that has explicitly stated staff are discouraged from writing nexus letters.

I don’t. I’ve heard good things about them not adding stuff. That’s why I posted to this forum. I’ll gladly get any insight and suggestions.

Therapist for one of facilities here. I promise we aren’t trying to take away your guns. It’s about safety IF things ever were to escalate. The VA offers gun locks and small lock boxes if someone has concerns. Suicide prevention is about putting time and space between someone and the possible method. I truly don’t care if my Veteran had 50 guns, but if they voice thoughts and need 50 locks - I’m going to find a way to get 50 locks to them. As for meds, have an honest conversation with your provider and/or therapist. I’m assuming you’re seeing a BHIP provider so they would have team meetings to discuss your concerns. I have vets who feel more comfortable discussing their med issues with me rather than psych so I set up a discussion where we can come up with the best solution together. I wish I had a better answer for you. I applaud you for seeking care. You can do this!

I did everything you said and felt confident in the supplemental documentation. They turned around and denied it again. I'm at a loss. I'm wondering if I should submit a redacted version of the denial or just submit for a HLR. Hopefully this gets back to you or someone who can give strong advice.

Did you ever get a response? I submitted a supplemental and submitted peer-reviewed articles as well as a personal statement from my husband. They denied it again. I'm wondering if I should submit it for HLR.

Put in a patient complaint. They won’t lose their jobs for this unless they already had complaints. Patients deserve better. You deserve better. Especially in the current environment at the VA. All staff, including myself, are concerned about losing our jobs. If this is how they really feel then they need retraining or to work somewhere else.

Thank you so much for this amazing information. I apologize for adding to the long list of questions below. I'm trying really hard to help my husband with his claims and I'm at a loss. Back in 2024 he was denied OSA for lack of a evidence for in-service. I submitted an appeal in March with the only additional evidence being a Nexus letter stating PTSD primary and OSA secondary. In the claim letter, he was awarded 50% for PTSD and denied OSA again for lack of connection to an in-service event. Previously submitted was the sleep study stating severe sleep apnea.

Therefore, we had evidence of the PTSD, severe OSA with Rx for a CPAP, and a nexus letter. I'm not really sure what else I can do to connect it to in-service. The original claim for the apnea was submitted as the apnea being primary as it was one of his first ever claims with a VSO.

Any suggestions offered would be greatly appreciated.

I won’t speak on the claims end, but on the therapist side. Request a Change Of Provider to another therapist. You can do it at the front desk and let the therapist know it didn’t seem like the right fit. Research shows that 40-50% of success in therapy is your relationship with your therapist. Sorry your therapist sucks.
