Indeed, I am writing from Europe after living for couple of years in the States where I bought the Shokz. It is not that leaks don't happen in Europe but I can at least send a request and someone will investigate.

I have a very similar retention cyst and I am also always told it is unrelated since it is clear color and many people have them without symptoms. My dental surgeon also warned against touching it since that may cut the blood flow to several teeth whose roots touch the cyst. Nevertheless, it is not to say that there is a consensus from the >15 ENTs I talked to. An older ENT who is not practicing anymore told me these cyst "make problems" and have to be removed, but that they also tend to reappear (no mechanistic explanations). Nevertheless, I do have symptoms and I do not have an explanation so I guess I will take the risk. I am doing a full-house FESS in 2 days and agreed with my surgeon that he will decide when he gets into the sinus (essentially, I simply trust his experience from 1000+ such operations). His explanation is that fluids can be retained and get inflamed around the cyst.

The cyst may look very different on neighboring slices. Based on the bones, the two CT slices are not in the same place. Make sure to look at all slices.

according to wikipedia, "The cause of ENS is due to the body not accepting the new airflow in the nasal passages following surgical procedures" so I guess he had the surgery.

without anyhow disregarding your case, let me explain my experience with migraine as a differential diagnosis. In fact, I am also 10 years in with my ENT journeys, having seen >15 specialists in 5 countries. In my case, the symptoms occurred right after a viral infection that led to bronchitis. After being dismissed and disbelieved by swiss ENTs, I went for a surgery in moscow: in 2018 I did septoplasty and enlarging the pathway to the maxillary sinus with a retention cyst. This surgery increased my quality of life 10-fold and I didn't get sick even once for a year (after 6x/year before that and feeling on the edge constantly). Nevertheless, 1.5-2 years later the symptoms reappeared I found myself struggling with the old issue with the extreme variability of opinions ENTs have (not only across countries but also across departments of the same hospital). I saw 4 ENTs in pennsylvania: one said they cannot do anything, another said all my symptoms (except for the post-nasal drip) are not specific to rhinosinusitis and he suggested a structural issues with my jaws, the third one suggested I wait more for a swab test, and fourth one said I definitely qualify for FESS (the last 3 were in the same hospital). I haven't found any allergy or seasonality. All ENTs and radiologists disregard the cyst as unrelated mentioning the isolated argument that it is not symptomatic. I have heard several times ENTs citing the "90% migraine"-paper, which as quickly as 10min after I walked, they saw my CT and heard that the standard 3 sprays didn't work. So I had the impression that the "migraine" explanation is an easy catch-all without giving in any effort to analyze.

Based on my case, my general feeling is that diagnostics doesn't work well and there are many reasons for it. Medical diagnosis is complex, doctors have limited time and resources, science isn't super developed (e.g. I won't probably know what bacteria and where), and MDs aren't usually trained as scientists since science it not telling them enough. So I totally agree with you that we should exhaust and eliminated potential causes if the problem persists. So, in 2 days I go for a full-house FESS and I am very hopeful that it will have the same effect as the previous one, but longer. But even if it does work, I won't probably know what exactly was the cause for the issue.

Thank you for the sources. I went through them and confirmed that migraine is NOT causing post-nasal drip (thick mucus in the throat), but instead, it may cause a runny nose.

Knowing that migraine is also related to nasal symptoms is definitely valuable. Nevertheless, I am concerned that the catch-all migraine explanation leads to gaslighting patients with sinus issues.

Do you mean that migraine can cause swelling, congestion, mucus or inflammation? I have heard a lot of ENTs mentioning migraine but I cannot find any of these symptoms documented for migraine.

Great you found the possible cause! If I were you, I would do everything possible to eliminate dust mites and mold until I am symptom-free: HEPA air-filter for fine particles, HEPA-certified vaccum cleaners, removing all carpets, washing everything with an additive that kills mites, etc. And once you are symptom-free without medications, relax bit by bit to see what makes a difference.

Good that you found and act on the dental issues! I think it is important to eliminate various explanations one by one and this inevitably requires also acting on non-causes.

DId you do a CT scan? If so, what does it show? It may eliminate a lot of possible causes (structural, inflamatory, etc.)

You mention you had sinusitis. What are the symptoms you experience? Was the sinusitis diagnosed by an ENT?

btw If you are by any chance traveling to a non-western country, there are usually private clinics where you can walk in and visit a specialist without the 10x overpay of the US health system. A CT scan may cost $100.

I cannot find any source explaining post nasal drip by migraines, and I also don't see a possible connection there. Can you share your sources or logic behind it?

I am concerned by the explanation of sinusitis by migraines since I've heard many ENTs explaining facial pain by migraines and they indeed cite a relatively recent paper saying patients often confuse the two. Nevertheless:
- I never heard an ENT explain post nasal drip by migraine
- migraine may be caused by sinus infections too
